Objective:Analyze the influencing factors of missed diagnosis of renal tumors by ultrasound to improve the ultrasound detection rate of renal tumors.Methods:Retrospective collection of 2033 patients with complete postoperative pathological diagnosis of renal tumors at Shanxi Provincial Cancer Hospital from January 2013 to October 2021.Based on the pathological results,renal tumors were divided into renal parenchymal tumors and renal pelvis tumors.Based on the preoperative ultrasound diagnosis results,the renal parenchymal tumors and renal pelvis tumors were divided into detection group and missed diagnosis group,respectively.Analyze factors such as ultrasound imaging features,CT or MRI images,and clinical data in each group,calculate the ultrasound missed diagnosis rate,and analyze the influencing factors of ultrasound missed diagnosis of renal tumors based on chi square test and multivariate logistic regression.Results:1.The rate of missed diagnosis of renal tumor by ultrasound was 1.87%(38/2033).2.The rate of missed diagnosis of renal parenchymal tumors by ultrasound was1.33%(25/1874).The results of univariate analysis showed that the missed diagnosis of renal parenchymal tumors by ultrasound was related to BMI,lesion size,side,location in upper pole/body/lower pole,ventral/dorsal side,and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that BMI≥24kg/m~2,small lesion,left,body and dorsal lesion were independent influencing factors for missed diagnosis of renal parenchymal tumors by ultrasound(P<0.05).3.The rate of missed diagnosis of renal pelvis tumor was 8.18%(13/159).Univariate analysis showed that missed diagnosis of renal pelvis tumor was related to BMI,gender,whether patients had ureteral or(and)urothelial carcinoma of bladder,lesion size,and lateral classification,with statistical significance(P<0.05).Multivariate Logistic regression analysis showed that BMI≥24 kg/m~2,ureteral or bladder uroepithelial carcinoma and small lesion were independent factors for missed renal pelvis tumor diagnosis by ultrasound(P<0.05).Conclusion:The influencing factors of missed diagnosis of renal parenchymal tumors by ultrasound are BMI≥24 kg/m~2,small lesion,located on the left side,body and dorsal.The influencing factors of missed diagnosis of renal pelvic tumors by ultrasound are BMI≥24kg/m~2,combined with other urinary system tumors and small lesions.
