ObjectiveThe study mainly explored the effects of ear point pressing beans,abdominal massage,and their combination on constipation symptoms and quality of life in patients with brain injury,and analyzed and compared the therapeutic effects of ear point pressing beans,abdominal massage,and their combination.To provide experimental basis for clinical nursing practice to screen safe,effective and economical Traditional Chinese medicine methods for treating constipation after brain injury.Method160 patients can be selected and divided into four groups by admission time,including control group,ear point pressing bean group,abdominal massage group and ear point pressing bean abdominal massage group,with 40 cases in each group.The patients in control group received routine intervention,and routine nursing methods of neurosurgery were used.On the basis of routine nursing,ear point pressing bean group was intervened by ear point pressing bean method.The patients in abdominal massage group were intervened by abdominal massage on the basis of routine nursing.On the basis of the control group,patients in the ear point pressing bean abdominal massage group were intervened by the ear point pressing bean method with abdominal massage method.The research tools include PAC-QOL,Somatic Self-rating Scale,Bristol stool form scale and general information questionnaire.The clinical efficacy,clinical symptom score of constipation,Bristol stool trait typing,CSBM,SSS,PAC-QOL score and blood pressure level in the four groups after intervention were compared.The VAS score before and after treatment was evaluated and compared.The total colon transit time,left colon transit time,rectal sigmoid colon transit time and right colon transit time were recorded,and the serum levels of IL-6,TNF-α,IL-8,motilin MLT,vasoactive intestinal peptide VIP and substance P were determined.The data were analyzed by using the analysis of variance,X~2-test and t-test in SPSS22.0 software.Result1.Symptoms:After intervention,the Bristol stool trait classification and complete spontaneous bowel movement(CSBM)frequency of patients in the combined group were significantly better than those in the control group,ear point pressing group,and abdominal massage group(X~2=29.335,X~2=20.332,P<0.05);The Bristol stool trait classification and CSBM frequency of patients in the ear point pressing bean group and abdominal massage group after intervention were significantly better than those in the control group(P<0.05);After intervention,there was no statistically significant difference in Bristol stool trait typing and CSBM frequency between the ear acupoint pressing bean group and the abdominal massage group(P>0.05).2.Efficacy:The total effective rate in the ear point pressing bean abdominal massage group was significantly lower than that in control group,ear point pressing bean group and abdominal massage group(X~2=30.508,P<0.05).The total effective rate in auricular point pressing bean group,and abdominal massage group was significantly lower than that in control group(P<0.05).There was no significant difference on the total effective rate between ear point pressing bean group and abdominal massage group(P>0.05).3.Gastrointestinal hormones and intestinal activities:The whole colon transit time,left colon transit time,rectsigmoid colon transit time and right colon transit time in ear point pressing bean abdominal massage group after intervention were better than those in the control group,auricular point bean pressing group and abdominal massage group(F=5.493,6.693,6.492,7.132,P<0.05).After the intervention,motilin MLT,vasoactive intestinal peptide VIP and substance P in ear point pressing bean abdominal massage group were better than those in control group,auricular point bean pressing group and abdominal massage group(F=8.580,7.602,7.443,P<0.05).4.Inflammatory index:There was no statistical difference on serum IL-6,TNF-αand IL-8 after intervention in each group(F=1.302,1.037,1.017,P>0.05).5.Quality of life:The PAC-QOL score of patients in the ear point pressing bean abdominal massage group after intervention was significantly higher than that in the control group,ear point pressing bean group and abdominal massage group(F=8.563,P<0.05).The PAC-QOL scores of the patients in the ear point pressing bean group and abdominal massage group after intervention were significantly higher than those in the control group(P<0.05).There was no significant difference on PAC-QOL score between ear point pressing bean group and abdominal massage group after intervention(P>0.05).6.Pain level and blood pressure level:The VAS score in ear point pressing bean abdominal massage group after intervention was significantly lower than that in control group,ear point pressing bean group and abdominal massage group(F=6.241,P<0.05).The VAS scores in ear point pressing bean group and abdominal massage group after intervention were significantly lower than those in control group(P<0.05).There was no significant difference in VAS score between ear point pressing bean group and abdominal massage group after intervention(P>0.05).There was no statistically significant difference in the average systolic and diastolic blood pressure among the four groups of patients 7 days after treatment(F=1.153,F=1.352,P>0.05).7.Psychological symptoms:The SSS score in the ear point pressing bean abdominal massage group after intervention was significantly better than that in control group,ear point pressing bean group and abdominal massage group(F=7.593,P<0.05).The SSS score in the ear point pressing bean group and abdominal massage group after intervention was significantly better than that in control group(P<0.05).8.In terms of expenses:The cost of treating constipation in the combination of ear acupoint pressing beans and abdominal massage group was significantly reduced compared to the control group,ear acupoint pressing beans group,and abdominal massage group(F=12.053,P<0.05).Conclusion1.The combination of ear acupoint pressing beans and abdominal massage has the good therapeutic effect on constipation in patients with traumatic brain injury,which can regulate the gastrointestinal hormone level of patients,promote intestinal activity and improve the patient’s stool characteristics.Its intervention effect is better than that of single ear acupoint pressing beans or abdominal massage intervention,and single ear acupoint pressing beans or abdominal massage intervention can also improve the constipation symptoms of patients with traumatic brain injury.However,the treatment effect of single ear acupoint pressing beans and abdominal massage on constipation in patients with traumatic brain injury is similar.2.The combination of ear acupoint pressing and abdominal massage intervention can improve the prognosis of patients with constipation after craniocerebral injury,help improve their quality of life,and improve their psychological status.3.The combination of ear acupoint pressing and abdominal massage intervention can alleviate the pain level of constipation patients after craniocerebral injury,and will not affect the patient’s blood pressure.And it can save the cost of treating constipation for patients with brain injury.
