| Objective: The aim of this study is to design a feasible and effective conservative treatment plan for mild to moderate adolescent idiopathic scoliosis,improve the patient’s curve and function,and delay or even avoid surgery.By studying the clinical efficacy of traditional Chinese medicine bone setting massage combined with Schroeder therapy,as well as its impact on maximum Cobb angle,apical vertebral rotation(Nash Moe method),total score of SRS-22,and various scores,we aim to explore the safety,effectiveness,and synergistic mechanism of the combination of the two.Methods: 1.Firstly,a case selection criteria was established,and60 suitable cases that met the established selection criteria were selected.They were randomly divided into a control group and a treatment group with 30 patients in each group.Statistical comparison of general data between the groups was conducted to prove that there was no difference.2.After excluding cases that do not meet the inclusion criteria,group treatment will be carried out.The subarm thoracolumbosacral brace was selected as the basic treatment for both groups,and the effective duration of wearing the brace should reach more than 20 hours per day.The control group: on the basis of wearing braces,the patients carried out specific physical therapeutic scoliosis exercises according to the type,mainly Schroess gymnastics,including the guidance of daily life posture,simple three-dimensional program,muscle cylinder movement,frog on the pond,handle movement,60~90 minutes of training each time,training at least twice a week,three months as a course of treatment,continuous treatment for six months.Treatment group: On the basis of wearing braces and performing Schroeder gymnastics,traditional Chinese medicine bone setting massage techniques were used.First,different massage techniques were targeted at the concave and convex sides,and then bone setting operations were performed.The main treatment was to treat the concave vertebral body.According to the type of scoliosis,the prone thoracic spine rotation partial pressure method,the prone posterior extension side top method,and the lumbar spine oblique pull method were used.Personalized outpatient orthopedic massage should be performed at least twice a week,with a 3-month course of treatment lasting for 6months.3.Before and after treatment,register and score,record the original data before treatment and the improvement data after treatment,including the Cobb angle,vertex vertebral rotation,and score values of the 22 items questionnaire of the Scoliosis Research Society in the standing position without braces before treatment,3 months after treatment,and 6 months after treatment.Use SPSS 26.0 software to statistically and analyze each data,and draw corresponding conclusions before evaluating the research results.Result: 1.Cobb angle: Analyzing the changes in Cobb angle before and after treatment in both groups(P<0.01),the results showed that the Cobb angle in both groups improved compared to before treatment,and the longer the treatment time,the more improvement in Cobb angle.There was no significant difference in Cobb angle between the two groups after 3 months of treatment(P>0.05),but there was a significant difference in Cobb angle after 6 months of treatment(P<0.05),indicating that the treatment group had better efficacy in improving Cobb angle in the longer term treatment.2.Apical vertebral rotation: In terms of apical vertebral rotation before and after treatment in both groups,the treatment group showed a significant improvement after 6 months of treatment compared to before treatment(P<0.01),while the control group showed no significant difference before and after treatment(P>0.05).After 3 months of treatment,there was no significant difference in the rotation of the apical vertebral body between the two groups(P>0.05),while after 6months of treatment,there was a significant difference in the rotation of the apical vertebral body between the two groups(P<0.05),indicating that the treatment group was better than the control group in improving the rotation of the apical vertebral body in the longer term of treatment.3.Total score of SRS-22 and various scores: In terms of total score of SRS-22,there was a significant difference between the two groups before and after 6 months of treatment(P<0.01),indicating that the total score of SRS-22 in the two groups after 6 months of treatment was significantly improved compared to that before treatment.In terms of other scores,after 6 months of treatment,there was no significant difference between the two groups in terms of functional activities and self image perception(P>0.05),while there were significant differences between the two groups in terms of SRS-22 total score,mental health,pain perception,and treatment satisfaction(P<0.01).It was stated that after the end of treatment,the treatment group had better scores in these four aspects than the control group.4.Clinical efficacy: Comparison of clinical efficacy between the two groups after 6 months of treatment(P<0.01),with a significant improvement rate of 63.33% in the treatment group compared to 20.00% in the control group.This indicates that the treatment group has better clinical efficacy than the control group,and can improve Cobb angle and alleviate symptoms to a greater extent.All 60 patients had no adverse reactions before and after treatment,such as increased back pain,limb numbness,chest tightness,palpitation,nausea,vomiting,dizziness,and headache.It can be seen that the treatment schemes in both groups have certain safety.Conclusion:1.Traditional Chinese medicine bone setting massage combined with Schroth therapy and Schroth therapy can improve patients’ Cobb angle and the apical vertebral rotation in terms of imaging manifestations,and the combination of the two has a better long-term effect.2.Traditional Chinese medicine bone setting massage combined with Schroth therapy and Schroth therapy can both improve the total score and various scores of patients with SRS-22.The combination of the two is better in improving the total score of SRS-22,pain perception,mental health,and treatment satisfaction.3.In this study,all patients did not experience any significant adverse reactions,and no patient underwent surgical treatment due to treatment failure,indicating that both therapies have a certain degree of safety in this study. |
