Research On The Construction Of Community Health Service Center Under Hierarchical Medical System

The establishment of hierarchical diagnosis and treatment system is an important part of deepening medical reform,which plays an important role in optimizing the allocation of medical resources and alleviating the problem of "difficult and expensive medical treatment" for the people.At present,our country continues to increase the construction of hierarchical diagnosis and treatment system,but the pattern of the "inverted triangle" of medical service system has not been fundamentally reversed,the phenomenon of "overcrowded" hospitals and "sparse" primary medical institutions remains.Under the hierarchical diagnosis and treatment system,community health service centers play an indispensable role in the first diagnosis and two-way referral at the grassroots level,and the construction of community health service centers is the focus of promoting the hierarchical diagnosis and treatment system.At present,there is still a gap between the construction of community health service centers and the requirements of hierarchical diagnosis and treatment system,especially in the central and western minority areas,this gap is more obvious.Through this study,it can provide reference for the construction of community health service centers in less developed areas in central and western China,especially ethnic areas,and has important theoretical and practical significance for enriching the management theory of community health service,improving the medical service capacity of community health service centers,rationally allocating medical and health resources,and promoting the establishment of hierarchical diagnosis and treatment system.Based on the theory of community health service management and the theory of new public service,combined with literature collection and questionnaire data analysis,this thesis focuses on the difficulties and main problems in the construction of A community health service center in Tongliao under the hierarchical diagnosis and treatment system,and puts forward countermeasures and suggestions based on the experience in the construction of community health service centers at home and abroad.The construction status of A community health service center was understood through field visits and reference to its relevant information.Through the distribution of questionnaires to community residents to understand the implementation of the hierarchical diagnosis and treatment system,and combined with the questionnaire survey data,summarized the problems in the center under the hierarchical diagnosis and treatment system.This thesis proposes that the construction of community health service center should be based on system,infrastructure,talents and ideas.In terms of system construction,institutional barriers between medical institutions at all levels should be broken around the requirements of "first diagnosis at the grassroots level" and "two-way referral" in the hierarchical diagnosis and treatment system,so that patients can be transferred orderly between superior and subordinate medical institutions.Meanwhile,supporting medical insurance administration and patient file information should also be seamless.In terms of infrastructure construction,the government should strengthen guidance,increase financial input,gradually improve and upgrade the medical equipment of community health service centers for diseases with a large number of patients,such as common diseases,frequently-occurring diseases and chronic diseases.In the construction of the talent team,the focus should be on the matching and strengthening of general practitioners and family doctors,and the structure of the talent team should be optimized to attract outstanding talents to serve in community health service centers.In terms of the construction concept,patients’ needs should be centered,advanced experience at home and abroad should be learned and used for reference,passivity should be turned into initiative,and changes should be actively sought.Publicity and information construction of the content of hierarchical diagnosis and treatment system should be strengthened.
