Objective To study the occurrence of anxiety and depression in patients with benign paroxysmal positional vertigo(BPPV,also known as otolithsis)and its influencing factors.Methods From January 2022 to December 2022,300 patients diagnosed with BPPV in the Department of Neurology of Baotou Central Hospital of Inner Mongolia Medical University with complete basic information were selected,and met the diagnostic criteria,enrollment criteria,and exclusion criteria.The study doctor briefly introduced the study orally and obtained the informed consent of the patient and his family.The Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD),which are widely used in the world,were used for survey analysis.At the same time,by collecting the patient’s gender,age,education level,length of course of vertigo attack,previous history of physical diseases(posterior circulation ischemia,migraine,Meniere’s disease,purulent otitis media,sudden deafness,craniocerebral trauma,previous otoliths,and other diseases),history of underlying diseases(normal presence of hypertension,diabetes,coronary heart disease,hyperthyroidism,cerebral infarction,cervical spondylosis or other diseases),whether there were sleep disorders,fatigue,mood disorders,Negative events and other influencing factors that may predispose BPPV patients to anxiety and depression were collated and statistically analyzed,so as to obtain independent risk factors that make BPPV patients prone to anxiety and depression.Results 1、Among the 300 cases,90 cases(30.00%)had abnormal HAMA score(HAMA>7),and 48 cases(16.00%)had abnormal HAMD score(HAMD≥8).Among them,there were 28 patients with definite anxiety(HAMA>14),and 3 patients with moderate to severe depression(HAMD>20).2、By comparing HAMA scores of BPPV patients with different characteristics,there were statistically significant differences in HAMA scores of 11 observation indicators(P<0.05).Further linear regression analysis was performed,and the results showed that the HAMA scores were correlated with the patient’s age,gender,course of disease,previous history of Meniere,pre-onset mood disorder,sleep disorder and fatigue state(B>0,P<0.05).By comparing HAMD scores of BPPV patients with different characteristics,HAMD scores of 9 observation indicators showed statistically significant differences(P<0.05).The linear regression model was further selected for analysis,and it was concluded that the HAMD scores were correlated with the patient’s age,gender,course of disease,pre-onset mood disorders and sleep disorders(B>0,P<0.05).3、HAMA scores and HAMD scores of different types of BPPV patients had statistical significance(P<0.05),and the right posterior semicircular canal group were significantly higher than other types.Conclusion 1、Patients with BPPV are at risk of depression and anxiety.2、Age above 50 years old,female,longer course of disease,previous medical history of Meniere’s disease,preoperative emotional disorders,sleep disorders and fatigue were the independent risk factors affecting HAMA scores in patients with BPPV Age over 50 years old,female,longer course of disease,comorbid mood disorders before onset,and comorbid sleep disorders were independent risk factors affecting HAMD scores in patients with BPPV 3、Different types of BPPV patients can be accompanied by anxiety and depression,but patients with right posterior semicircular canal otolithiasis are more likely to cause changes in anxiety and depression.
