Objective This study studied cognitive reserve(CR)proxy variables of patients with Alzheimer’s disease(AD)in the pre-dementia stage:Education,work activities and leisure time were used to determine the relationship between CR and the comprehensive cognitive domain of AD patients in the pre-dementia stage,and to explore the protective effect of CR in the brain.Methods Select physical examiners,outpatients,or inpatients who visited the Department of Neurology at Baotou Central Hospital from September 2020 to November 2022,including 25 normal control(NC),28 patients with subjective cognitive impairment(SCD),and 45 patients with mild cognitive impairment(MCI).All enrolled patients were evaluated with the Cognitive Reserve Index questionnaire(CRIq),which generated total scores,educational level,work activity,and leisure time scores(CRI-Total,CRI-Education,CRI-Working activity,and CRI-Lisure time,respectively).Detailed cognitive function scales were also evaluated.Finally,correlation analysis and linear regression analysis were conducted on the proxy variables of cognitive reserve and the scores of each cognitive domain of the cognitive function scale.Results 1.Cognitive function: Compared with the NC group,the SCD group showed a statistically significant decrease in scores on stroop color word test C,digit span forward test,and Boston naming test(P<0.05),while there was no statistically significant decrease in other cognitive functions;The cognitive function scores of the MCI group were generally lower.2.Correlation between CR and cognitive function:(1)Correlation between CR and comprehensive cognitive function score: CRI-Total score,CRI-Education score,CRI-Leisure time score positively are correlated with MMSE score and Mo CA score.(2)The correlation between CR and detailed cognitive function score: The CRI-Education score is positively correlated with the symbol digit modalities test,the total score and delayed recall of auditory verbal learning test,and Boston naming;The CRI-Working Activity score is positively correlated with the symbol digit modalities test,auditory verbal learning test delayed memory,and digit span forward test;The CRI-Leisure time score is positively correlated with the total score and delayed recall of auditory verbal learning test,digit span forward test,symbol digit modalities test,animal verbal fluency,and Boston naming;The CRI-Total score is positively correlated with the total score and delayed recall of auditory verbal learning tests,digit span forward test,animal word fluency test,symbol digit modalities test,and digit span backward test.Conclusion 1.There is a correlation between the comprehensive cognitive reserve and overall cognitive function in patients with Alzheimer’s disease in the pre dementia stage.2.There is a correlation between educational level and comprehensive cognitive scale,memory,and language ability;The role of occupational complexity in improving cognition is not significant,as it is related to the comprehensive cognitive scale and memory;There is a correlation between lifelong leisure activities and comprehensive cognitive scales,memory,attention,and language abilities.3.High CR levels can improve the cognitive function of patients in the pre dementia stage of AD and delay the occurrence of dementia.
