Objectives:(1)To explore the correlation between TyG index and carotid atherosclerosis in AIS patients;(2)To explore the correlation between TyG index and the stability of cervical atherosclerotic plaque and the degree of vascular lumen stenosis in AIS patients,and To analyze the diagnostic value of TyG index in carotid unstable plaque.Methods: General clinical data and CTA imaging data of patients diagnosed with AIS in the Department of Neurology of the First Affiliated Hospital of Dali University from January2021 to July 2022 were retrospectively analyzed.A total of 225 cases were included.According to the results of CTA examination,those without carotid atherosclerosis were classified as the control group(n=63),and those with carotid atherosclerosis were classified as the observation group(n=162).Then the observation group was divided into low score group(TyG index <8.98,n=93)and high score group(TyG index ≥ 8.98,n=69).The TyG index and clinical laboratory data between the observation group and the control group and between the observation group subgroups were compared and analyzed.The risk factors of carotid atherosclerosis and unstable plaque in AIS patients were analyzed by unconditioned Logistic regression.Finally,ROC curve was used to evaluate the value of TyG index in the diagnosis of carotid atherosclerosis with unstable plaque,and to determine the best diagnostic cut-off value of TyG index.Results:(1)There were no significant differences in gender,smoking history,drinking history,TC,LDL-c and BMI between the observation group and the control group(P > 0.05).TyG index level of the observation group 8.89(8.50~9.31)was higher than that of the control group 8.52(8.40~8.79),the age observation group(63.96±10.45)was higher than that of the control group(51.86±15.60),HDL-c The level of 1.13(0.92~1.35)in the observation group was lower than that of 1.23(1.02~1.52)in the control group,and the difference was statistically significant(P < 0.05);The difference between the observation group and the control group in the history of diabetes and hypertension was statistically significant(P < 0.05).Multivariate logistic regression analysis showed TyG index(OR=2.257,95%CI: 1.118~4.559),age(OR=1.085,95%CI: 1.053~1.119)and hypertension(OR=2.631,95%CI: 1.272~5.493)were independent risk factors for cervical atherosclerosis in AIS patients.HDL-c(OR=0.309,95%CI: 0.106~0.907)is a protective factor for cervical atherosclerosis.(2)TyG index high score group and low score group had no statistical significance in hypertension,smoking,drinking history and degree of stenosis(P > 0.05).;There were statistically significant differences in age,gender,diabetes history,TC,LDL-c,HDL-c,BMI,TyG index and plaque stability between the two groups(P < 0.05).(3)Univariate Logistic regression analysis showed that diabetes history(OR=2.275,95%CI:1.100~4.407),smoking history(OR=2.226,95%CI: 1.187~4.175),HDL-c(OR=0.184,95%CI:0.064~0.528)and TyG index(OR=5.887,95%CI: 2.964~11.694)were associated with carotid artery unstable plaque.Multivariate Logistic regression analysis showed that TyG index(OR=5.499,95%CI: 2.552~11.850)and smoking(OR=2.357,95%CI:1.127~4.930)were independent risk factors for atherosclerotic unstable plaque in the neck of AIS patients.(4)The analysis results of ROC curves showed that the optimal truncation value of TyG index in the diagnosis of cervical atherosclerotic unstable plaque in AIS patients was 8.995,the area under the curve was 0.753,the sensitivity was 60.0%,and the specificity was 81.7%.Conclusion:(1)TyG index is associated with carotid atherosclerosis in patients with AIS,and high TyG index is an independent risk factor for cervical atherosclerosis in patients with AIS.(2)TyG index was associated with carotid atherosclerotic unstable plaque in patients with AIS,and high TyG index was an independent risk factor for carotid artery unstable plaque in patients with AIS..(3)TyG index was not correlated with the degree of carotid artery stenosis in patients with AIS.
