Psychological Status Of Patients With Colorectal Polyps And Associated Influencing Factors

Objective:This study investigated the status of anxiety and depression symptoms in patients with colorectal polyps and explored the associated influence factors,for providing evidence and clues to promote the diagnosis and treatment of psychosomatic diseases of the digestive system.Method:1.A total of 507 patients who underwent colonoscopy and found colorectal polyps in seven tertiary hospitals of Xinjiang Production and Construction Corps from December 2021 to June 2022 were enrolled.The Generalized Anxiety Disorder Scale 7(GAD-7)was used to screen for anxiety symptoms,the Patient Health Questionaire 8(PHQ-8)was used to screen for depression symptoms,and the general situation questionnaire was used to collect clinical information of patients.2.The cases were collected by the questionnaire Star platform and a database was established.SPSS 25.0 statistical software was used for statistical analysis.For continuous variables(non-normal distribution),median and quartile spacing were used,and a non-parametric rank sum test was used for comparison between groups.Categorical variables were expressed as n(%).Theχ~2 test or Fisher’s exact test was used to compare the difference between groups.The rank-sum test was used to compare the ordered data groups.Univariable analysis was performed by binary unconditional Logistic regression,and variables with P<0.1 in univariable analysis were included in multivariable Logistic regression analysis.Independent risk factors were analyzed by multivariable Logistic regression.P<0.05 was considered statistically significant.Result:1.The detection rate of anxiety in patients with colorectal polyps was 21.50%.15.78%of them had mild anxiety symptoms,2.96%had moderate anxiety symptoms,1.97%had moderate to severe anxiety symptoms,and 0.79%had severe anxiety symptoms.Univariable unconditional Logistic regression analysis of anxiety showed that sex,BMI,marriage,perception of polyps,times of polyps detected,polyp size,polyp number,abdominal pain,abdominal distension,diarrhea or constipation symptoms,number of gastrointestinal symptoms,and time of gastrointestinal symptoms onset may be the influencing factors of anxiety symptoms(P<0.1).The results of the multivariable Logistic model showed that female(OR=3.87,95%CI:2.30-6.61),polyp size(OR=1.74,95%CI:1.14-2.67),gastrointestinal symptoms onset after polyp detection(OR=5.43,95%CI:1.74-16.92),and the perception of polyps as cancer(OR=13.96,95%CI:1.48-132.07)were independent influencing factors for anxiety symptoms,and unclear perception of polyps(OR=0.49,95%CI:0.28-0.86)was a protective factor for anxiety symptoms.2.The detection rate of depressive symptoms was 19.33%.The detection rates of mild,moderate,moderate to severe,and severe depression were 13.81%,3.75%,0.79%,and 0.99%,respectively.Univariable unconditional Logistic regression analysis showed that sex,BMI,gastrointestinal cancer history in immediate family members,perception of polyps,the times and the number of polyps,abdominal pain,abdominal distension,diarrhea or constipation,the number of gastrointestinal symptoms,and the time of gastrointestinal symptoms onset may be the influencing factors of depressive symptoms(P<0.1).Multivariable Logistic regression analysis showed that female(OR=2.42,95%CI:1.47-4.00),the number of polyps(OR=1.07,95%CI:1.01-1.13)and the number of gastrointestinal symptoms(OR=2.04,95%CI:1.34-3.10)were independent influencing factors for depressive symptoms.Conclusion:This multicenter study reports for the first time that the detection rates of anxiety and depression symptoms in patients with colorectal polyps in the local area are high.It is necessary to focus on and early identify the negative psychological symptoms of women,large polyps,large number of polyps,wrong cognition of polyps,and new gastrointestinal symptoms after polyps detection.
