| Objective: This study explores the clinical efficacy of the rotary lifting technique in treating cervical spondylosis of nerve root type(Qi stagnation and blood stasis type)through clinical trials and human simulation modeling experiments and analyzes the mechanical mechanism of the rotary lifting technique on the human body.It provides a reference for its widespread application and promotion in clinical practice.Methods:(1)Part Ⅰ36 patients with cervical spondylosis of nerve root type(diagnosed as Qi stagnation and blood stasis type by TCM)who visited the orthopedic clinic of Changchun University of Chinese Medicine Affiliated Hospital from April to December 2021 were selected.Using central randomization method,with the help of central randomization system and given seed number,they were divided into experimental group and control group.The experimental group had 18 cases treated with rotary lifting technique + oral neck pain granules,while the control group had 18 cases treated only with oral neck pain granules.The efficacy was evaluated before and after treatment and one month after treatment in both groups.The evaluation indicators included VAS score,JOA score,NDI score.All patients underwent radiological examination once before and after treatment and curvature measurement was performed.(2)Part Ⅱ17 healthy adult males and 15 healthy adult females were recruited as modeling subjects.The subjects had no history of cervical spondylosis,scoliosis or strain.Motion capture was performed using Visual3 D software and inverse dynamics analysis was performed using the VICON three-dimensional infrared motion analysis system to establish a musculoskeletal model of the human body.Stress was applied to the front of the middle and lower trunk of the subjects to observe the changes in cervical curvature under different loading conditions.At the same time,force decomposition was performed on the entire cervical spine to analyze the relationship between changes in cervical curvature and loading.The radiological measurement results of two groups of patients collected in Study 1 were analyzed to derive their mechanical mechanism.Results:(1)Part ⅠThe baseline data(gender,age,BMI,duration of illness,etc.)and evaluation indicators before treatment(VAS score,NDI index,JOA score,cervical curvature)of the two groups showed no significant difference(P>0.05)and were comparable.Evaluation indicators:(1)VAS score: The difference between the two groups was statistically significant(P<0.05),and the difference between post-treatment and one month after treatment compared to pretreatment was statistically significant(P<0.05);(2)NDI score: The difference between the two groups was statistically significant(P>0.05);compared with pre-treatment at two time points after treatment,the differences were all statistically significant(P<0.05);(3)JOA score: The difference between the two groups was statistically significant(P<0.05),and the difference between post-treatment and one month after treatment compared to pre-treatment was statistically significant(P<0.05);In terms of imaging,cervical curvature CCD: After treatment,the depth of physiological curve of cervical spine in experimental group increased and the difference between two groups was statistically significant(P>0.05).(2)PartⅡAccording to the data,a relationship curve was plotted and a formula was derived.At the same time,force analysis was performed on the entire cervical spine.The results showed that when the stress on the front of the spine increased,the curvature of the cervical spine increased.According to the force analysis results,within a certain range of motion,when the curvature of the cervical spine increased,the shear force decomposed in the direction of cervical spine decreased and pressure on cervical spine decreased.In patients with cervical spondylosis without load-bearing status,when cervical curvature becomes straightened,shear force along tangent direction increases and approaches head gravity.At same time statistical results of radiological data in Study 1 were analyzed and it was found that after treatment mode of force on cervical spine changed.Conclusion: On the basis of drug therapy,lifting manipulation can better improve the pain and function of cervical spondylopathy patients,improve the quality of life of patients,and with the continuation of exercise time,the better the improvement effect;At the same time,the rotational lifting technique improves the cervical spine curvature significantly,and from the biomechanical analysis,it is concluded that the increase of cervical spine curvature can relieve the cervical spine pressure. |
