Objective(s): To analyze lumbar and hip bone mineral density(BMD)and osteoporosis in the middle-aged and elderly population in Kunming by quantitative computed tomography(QCT).Methods: From March to November 2021,middle-aged and older people aged(≥50)in Kunming who voluntarily received QCT of the lumbar spine and hip in Yunnan First People’s Hospital were selected,and follow-up(measurement of lumbar BMD)was performed in older adults ≥60 years of age at year 2.Their medical history,height and weight were recorded.The subjects were divided into three groups(50-59 years old;60-69 years old;Aged 70 and above)according to different genders with ten years old as an age level.Analyzed mean bone mineral density(L1-2 BMD)of lumbar 1-2 vertebra at year 1,total hip areal bone mineral density(TH a BMD),total hip volumetric bone mineral density(TH v BMD),femoral neck areal bone mineral density(FN a BMD),femoral neck volumetric bone mineral density(FN v BMD),trochanter areal bone mineral density(TR a BMD),trochanter volumetric bone mineral density(TR v BMD),intertrochanter areal bone mineral density,(IT a BMD),intertrochanter volumetric bone mineral density(IT v BMD)and Mean bone mineral density(L1-2 BMD)of lumbar 1-2vertebra at year 2.Results: Baseline :A total of 555 subjects(180 males and 375 females)were included.The overall prevalence of OP based on BMD measurement of lumbar spine,full hip and any part of lumbar spine were 33.51%,12.79% and 36.04%.There was no difference between the detection rate of OP in any amount and that of the lumbar spine(P > 0.05).The OP detection rate of the lumbar spine and any part was significantly higher than that of the hip(P < 0.001).In the 50-59 age group,there was no significant difference between male and female lumbar 1-2 BMD(P>0.05).The BMD of lumbar 1 and 2 in women aged 60-69 and over 70 years was lower than that in men(P<0.001).There was no correlation between BMD and age in male(P>0.05),but there was a negative correlation between BMD and age in female(P<0.05).TH v BMD,FN v BMD,TR a BMD and IT v BMD significantly differed between male and female in 50-59 age group(P<0.05),all except TR a BMD were higher than male.The TH a BMD,FN a BMD,TR a BMD,TH v BMD and IT a BMD of male in 60-69 age group were higher than those of female(P<0.05);In the 70 + age group,BMD of male hip was higher than that of female(P<0.05).Follow-up: A total of 123 elderly patients(40 males and 83 females)were included in the study.In the 60-69 age group,the lumbar vertebra density loss rate was 5.26% in males and 5.04% in females.The loss rate of lumbar vertebra density was 6.44% in males and 4.87% in females aged 70 years and above.Conclusion(s):(1)The prevalence of OP in middle-aged and elder people aged 50-59 years in Kunming was higher than that in plain areas,and the majority of OP in women ≥60 years was significantly higher than that in men,and the hip BMD in women was lower than that in men.(2)The overall prevalence rate of OP measured in the lumbar spine of middle-aged and elder people in Kunming was 33.51%,and the total hip was 12.61%.Only the lumbar spine can be measured when screening and diagnosing OP using QCT.(3)The rate of bone loss one year after baseline in women ≥60 years was lower than that in men,and the rate of bone loss tended to be stable.
