Objective: To explore the risk factors of stroke-associated pneumonia(SAP)in patients with acute ischemic stroke(AIS),in order to provide an objective basis for the prevention of SAP.Method: A total of 156 patients with AIS hospitalized in the Department of Neurology,The First Affiliated Hospital of China Medical University from July 2021to June 2022 were retrospectively collected,meeting the inclusion and exclusion criteria.These patients were divided into SAP group(61 cases)and non-SAP group(95 cases)according to whether they had concomitant SAP during hospitalization and within 7 days after AIS attacked.Basic information and clinical data of these patients in both SAP and non-SAP group were collected,including personal information,past medical history,clinical data,TOAST classification of cerebral infraction and stroke related scores.Univariate analysis was performed on the data of patients in SAP and non-SAP group,and then statistically significant indicators were introduced into binary Logistics regression to analysis the independent risk factors of SAP occurrence.The independent risk factors were analyzed by ROC curve.SPSS 26.0 were used for data analysis and graphing.Result: By unifactorial analysis,age≥75 years,history of coronary heart diseases,NIHSS score ≥10 at admission,increased NIHSS score after admission,disturbance of consciousness,dysphagia,restriction of activity,hypoalbuminemia,use of nasal feeding tube,and large artery atherosclerosis infarction(LAA)were associated with the occurrence of SAP(P<0.05).Logistics regression analysis showed that dysphagia(OR=29.992,P=0.004),hypoalbuminemia(OR=6.823,P=0.039),use of nasal feeding tube(OR=37.559,P=0.006),and LAA(OR=6.432,P=0.029)were independent risk factors for SAP(OR>1,P<0.05).Independent risk factors of continuous variables were analyzed by ROC curve.Hypoalbuminemia showed excellent predictive value for SAP(AUC=0.838>0.8,P=0.000<0.05).Conclusion: Dysphagia,hypoalbuminemia,use of nasal feeding tube,and LAA were independent risk factors for SAP.Among them,hypoalbuminemia has excellent predictive value for SAP.Intervention should be taken for AIS patients with above risk factors to improve the prognosis for patients.
