Objective:This study retrospectively analyzed the clinical data of 39 patients with oral and maxillofacial malignancies accompanied by multiple primary cancers.The aim was to investigate the clinical characteristics and prognosis of these patients and provide guidance for the diagnosis,treatment,and prognosis of this disease.Methods:The data were collected from patients admitted to the Oral and Maxillofacial Surgery Unit of Shengjing Hospital Affiliated to China Medical University between October 2019 and November 2022.Univariate analysis of the clinical characteristics of the patients was conducted using SPSS25.0 software,followed by multivariate analysis to identify the independent factors affecting the prognosis of patients with oral and maxillofacial malignancies accompanied by multiple primary cancers.Results:The results showed that among the 39 patients,34(87.1%)had double primary malignancies and 5(12.8%)had triple primary malignancies.The first site of malignancy was oral and maxillofacial in 5 patients(12.8%),while it was non-oral and maxillofacial in 34 patients(87.1%).Concurrent oral and maxillofacial malignancies with multiple primary cancers were present in 8 cases(20.5%),while heterogeneous oral and maxillofacial malignancies with multiple primary cancers were present in 31 cases(79.5%).Radiotherapy and chemotherapy were identified as independent prognostic factors for patients with oral and maxillofacial malignancies with multiple primary cancers(P<0.05).Cox multivariate analysis showed that radiotherapy and chemotherapy and treatment of primary cancer were independent influencing factors for patients with heterogeneous oral and maxillofacial malignancies accompanied by multiple primary cancers.The treatment of the second primary cancer was identified as an independent prognostic factor for patients with the second primary site located in the oral and maxillofacial region.Conclusion : The most common oral and maxillofacial malignancies with multiple primary cancers were heterogeneous oral and maxillofacial malignancies.The digestive system was the most commonly affected,followed by the respiratory system(lung),reproductive system(uterus),and mammary gland.Radiotherapy and chemotherapy were independent factors for the prognosis of oral and maxillofacial malignancies with multiple primary cancers.For patients with heterogeneous oral and maxillofacial malignancies with multiple primary cancers,failure to perform radical resection of the first primary cancer was an independent factor affecting the prognosis.For patients with the second primary site located in the oral and maxillofacial region,the failure to perform radical resection in the treatment of the second primary cancer was an independent factor affecting the prognosis.
