Objective: To study the effect of TMJ disc irreducible anterior disc displacement without reduction(ADDWOR)in TMJ disc anchorage(DA)with double-wire anchor and follow-up of 6 months,and analyze the influence of disc position,disc length,condyle height and age on the effect of operation.Methods: From June 2021 to February 2022,30 patients with TMJ disc irreducible anterior displacement diagnosed in our department of maxillofacial surgery and treated with double-wire anchors during TMJ disc anchorage(30 joints)were included in this study.Clinical results were evaluated by clinical examination and questionnaire survey of clinical results.Disc length and condylar height were measured by MRI to evaluate surgical results on imaging.Results: The mean age of the patients was26.30±7.60 years old,the mean oral opening was(29.17 ± 5.38mm)before operation(range: 19-42mm),the mean maximum oral opening was(39.17 ± 2.57mm)after operation(range: 35-45mm),10.00 ± 4.33 mm better than before operation,and the mean oral opening was more than 35 mm in 93.3% of the patients after operation;the VAS score was 58.33 ± 21.19(10-90)before operation,11.67 ± 6.99(0-20)at 6 months after operation,and the oral opening and VAS value at 6 months after operation were significantly different(P 0.05 <).Among 30 patients,22 patients were satisfied with self-evaluation,accounting for 73.33%,8 patients were basically satisfied with self-evaluation,accounting for 26.67%,none were dissatisfied.The disc length was 9.77 ±0.63 mm,11.96 ± 0.68 mm and 12.72 ± 0.55 mm at preoperative,immediate postoperative and 6-month follow-up.The height of condylar process was 11.63±1.46 mm and12.71±1.23 mm at before surgery and 6 months follow-up respectively.The condylar process increased 1.02±0.59 mm atbefore surgery follow-up.Conclusion: 1.The clinical symptoms and joint function of the patients were significantly improved after the operation;2.MRI confirmed the stability of the relationship between the articular disc and condyle.
