| Objective:This study mainly collected and collated the clinical records of patients with coronary heart disease caused heart failure,analyzed and summarized the distribution of traditional Chinese medicine(TCM)syndrome types,and the rules of prescriptions and medications of coronary heart disease caused heart failure,with the aim of providing more creative ideas and directions for the diagnosis and treatment of this disease in TCM.Methods:The collected data for this study were obtained from inpatients admitted to the Department of Cardiovascular Diseases of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from September 2020 to September 2022.By using Microsoft Excel to input patient related information and conducting frequency statistical analysis.By using IBM SPSS Statistics 26,a statistical analysis was conducted using appropriate statistical methods according to the characteristics of the statistical data.Cluster analysis was performed on high-frequency Chinese medicines to summarize common drug pairs and drug combinations.Drug association rules analysis was carried out using IBM SPSS Modeler 18 software.Results:1.In this study,there were 122 male patients and 103 female patients.The mean age of all patients was(71.49±9.87)years.The season of onset of the patients was predominantly autumn and winter.2.The included patients may have one or more combined diseases,with hypertension,arrhythmia and cerebrovascular disease ranking the top three.The top three high frequency used western medicine are diuretics,RAAS inhibitors,and β-receptor blocker.3.In this study,a total of 11 proprietary Chinese medicines were used by the patients included.Floium ginkgo extract and tertram ethypyrazine sodium chloride injection being the most frequently used.4.In this study,lilac tongue,dark purple tongue with petechiae,pink tongue,dull tongue were the most common tongue nature,and thin white fur and white greasy fur were the most common tongue coating.The pulse was most common with a stringy and slippery pulse and a sunken and unsmooth pulse.5.The frequency of TCM syndrome of the patients included in this study was: qi deficiency and blood stasis syndrome > phlegm-blood stasis syndrome > syndrome of water overflowing due to yang deficiency > qi and yin deficiency syndrome.6.In this study,the largest number of patients had class IV cardiac function.The LVEF values of patients with syndrome of water overflowing due to yang deficiency were significantly lower than that of the other groups.Patients with LVEDD values ≥ 60 mm are predominantly suffering from syndrome of water overflowing due to yang deficiency.7.A total of 225 traditional Chinese medicines were used in the prescriptions included in this study,with a total medication frequency of3850 times,among which those used more than 100 times included atractylodes,astragalus,poria cocos,cassia twig,angelica sinensis.8.In the statistics of TCM properties,the drugs with warm nature were used the most frequently,followed by drugs with cold nature.In the statistical analysis of medicinal flavours,the frequency of sweet,bitter and pungent medicines was high,with a cumulative frequency of 84.87%.The main meridians of the drug are the lung,spleen,liver,heart and kidney meridians,with a cumulative frequency of 74.90%.9.In this study,the clustering analysis of 32 high-frequency drugs yielded 9 groups of drug pairs and 4 groups of clustered combinations of prescriptions,of which oyster-keel was the drug pair with the highest frequency of occurrence.The analysis of association rules for 93 Chinese herbal medicines used ≥ 10 times resulted in a total of 45 groups of commonly used drug combinations.Conclusion:1.In this study,the number of male patients was slightly larger than the female patients,and there was a high incidence of elderly patients.2.In this study,the most common TCM evidence type in patients with coronary heart disease caused heart failure was qi deficiency and blood stasis syndrome.There was a correlation between cardiac function classification,LVEF,LVEDD and TCM syndrome.3.In this study,the common drug pairs were oyster-keel,semen persicae-safflower,fructus trichosanthis-allium macrostemon,ophiopogon japonicus-schisandra chinensis,ligusticum wallichii-red peony,bupleurumfructus aurantii,areca peel-agaric,aster-motherwort,and atractylodes-poria cocos.4.In this study,the core prescription for the treatment of coronary heart disease caused heart failure was astragalus-pseudostellaria-atractylodes-ang-elica sinensis-ligusticum wallichii-poria cocos-cassia twig-salvia miltiorrhi-za.This formula has the function of tonifying qi-blood,invigorating yin and warming yang,removing stasis and alleviating water retention. |
