The Relationship Between Visceral Fat And Lung Function,Exercise Tolerance,Inflammation Level In Patients With Stable Chronic Obstructive Pulmonary Disease(COPD) And The Change Rule Of TCM Syndrome Elements

Abstract/Summary:
Objective: To understand the effect of different visceral fat accumulation on patients with stable COPD by analyzing the body composition indicators such as visceral fat,lung function,serum inflammation and blood glucose related indicators and exercise endurance in patients with stable COPD.The bias of syndrome elements in the two groups was analyzed by the TCM syndrome elements table,so as to provide theoretical and data support for the diagnosis and treatment of COPD patients with lung-spleen qi deficiency.Methods: Patients diagnosed with stable COPD of lung-spleen qi deficiency in the Department of Respiratory and Critical Care Medicine of Xinjiang Traditional Chinese Medicine Hospital from 2022 to 2023 were collected.General clinical data,body surface measurement indicators,body composition,laboratory related indicators,lung function,m MRC,CAT score,6MWD,syndrome element score table,etc.The correlation and difference were compared.Results:(1).VFA decreased significantly with the increase of FEV1%Pred.(2)According to different VFA,the levels of FEV1%Pred and FVC%Pred in the high group were higher than those in the normal group and the high group;With the increase of VFA,m MRC score,CAT score and 6MWD showed a downward trend,and the results were not statistically significant(P>0.05).In the statistics of exercise habits,the normal group mainly exercised more than 3 times a week and never exercised,and the high group and the high level group had the highest proportion of never exercised.The three groups of exercise purposes were mainly healthy exercise,and the exercise method was mainly aerobic exercise.There was no significant difference between different VFA levels and inflammatory index levels,blood glucose and blood lipids(P >0.05).(3)There was a significant difference between VFA and FVE1/FVC% in female subjects(P <0.05).In the male group,VFA was significantly correlated with FVE1/FVC and FVC%(P<0.05).There were significant differences in FEV1% and FVC% among different age groups.VFA level decreased with age.(4)In the correlation analysis,BMI and FEV1%were positively correlated in the normal VFA group and negatively correlated in the high VFA group.(5)The main clinical symptoms were cough,shortness of breath,fatigue and exhaustion.The severity of TCM syndrome elements increased with the increase of VFA.Conclusions:(1)COPD patients with high body fat have higher degree of lung function limitation,which is an important risk factor for lung function decline.The higher the visceral fat area level,the lung function does not necessarily decrease,and may be a protective factor,and the trend in women is higher than that in men.(2)The severity of TCM syndrome elements was the lowest in the high group.It is recommended that COPD patients actively choose physical exercise and pulmonary rehabilitation exercise,appropriate weight control and monitoring of visceral fat.
