Prognostic Significance Of Neutrophil Lymphocyte Ratio And Red Blood Cell Distribution Width In Peripheral T-cell Lymphoma

Objective: To investigate the value of neutrophil / lymphocyte ratio(NLR)and Red cell distribution width(RDW)in patients with peripheral T cell lymphoma(PTCL).Methods: 50 newly diagnosed PTCL patients were retrospectively analyzed in the Second Affiliated Hospital of Anhui Medical University from December 2008 to December 2021,and were confirmed by lymph node biopsy pathology and immunohistochemistry.Clinical and laboratory data were collected: age,sex,Ann Arbor stage,ECOG score,IPI score,B symptoms,number of extranodal involvement,bone marrow involvement,absolute lymphocyte count,neutrophil count,RDW,albumin,lactate dehydrogenase and C reactive proteins.Treatment protocol used a CHOP or CHOP-like protocol.The ROC curve was used to determine the cut off values suitable for the RDW,NLR.The effects of NLR,RDW and other clinical and laboratory factors on total survival(overall survival,OS)and progression-free survival(progression free survival,PFS),spearman correlation with clinical characteristics,and unifactor and multifactor analysis of clinical and laboratory data on PTCL patients by Cox regression.The follow-up was completed by telephone or by checking the outpatient and inpatient medical records.Results :In 50 patients with PTCL,29 had Ann Arbor stage III and IV(58%),IPI greater than 2points(54%),and 24 received CHOP regimen(48%)and 26(52%)received CHOP-like regimen.The Cut-off values for the NLR and RDW were 4.6 and 13.75%,respectively,Median follow-up time was 18.5 months(February – 10 August).The NLR was significantly associated with the ECOG score(p <0.001),the IPI score(p <0.001),and the Ann Arbor stage(p <0.001),There was no significant correlation with age,sex,bone marrow invasion,and more extranodal lesions;Studies showed that RDW 13.75% was significantly associated with ECOG score(p<0.001),IPI score(p <0.001),Ann Arbor stage(p <0.001),and extranodal lesion site(p <0.001),However,there was no obvious correlation with age,gender,and bone marrow invasion.The Kaplan-Meier survival curve showed a shorter OS(p<0.001)between NLR 4.6 and 13.75% of RDW patients.Univariate and multivariate COX model regression analysis found that both higher NLR levels and higher RDW levels were OS-independent prognostic risk factors in PTCL patients(p <0.001).Conclusion: NLR and RDW can be used as independent prognostic indicators for naive PTCL patients;NLR,RDW level rise,in the CHOP,and CHOP sample treatment also lead to shorten the OS。...
