Objective:Based on the theory of active aging and hierarchy of needs,this study constructs the Namaste care programme for patients with advanced senile dementia and analyzes the clinical application effect of the plan,so as to provide theoretical support and practical guidance for improving the quality of life of patients with advanced senile dementia.Methods:The research method consists of two parts.The scheme was constructed using the literature research method to characterize the contents extracted from the literature review,the semi-structured interview method,and the expert group meeting method for content revision and feasibility evaluation,then two rounds of Delphi expert correspondence with 15 experts to revise and improve the entries according to the expert’s opinions,and finally the administration draft of the scheme was formed after revision by a pre-test.A mixed research method was used to analyze the application effect of the scheme.In the quantitative study,45 study subjects who met the criteria of nadir were conveniently selected in a tertiary care hospital in Sichuan Province,and the Namaste care programme was added to the conventional care,and questionnaires were conducted before,1 month,3 months and 6 months after the intervention to analyze the effect of the programme on quality of life,pain and aggressive behavior;in the qualitative study,the responsible nurses and primary caregivers who met the criteria of nadir were selected for semi-structured interviews immediately after 6 months of the intervention,and the data were analyzed using Colaizzi phenomenological analysis to further explain and supplement the application effect.Results:Part I: A total of 10 literature studies were included in the literature study,and the initial entries of the scheme were formed based on literature characterization;the first draft of the scheme was formed after modification based on the results of interviews with 11 nurses and feasibility evaluation by 6 experts.In the two rounds of expert consultation,the positive coefficients of experts were 100%,and the Kendall harmony coefficients(w)were 0.496 and 0.670,respectively(both p<0.001),and the final draft of the scheme was formed after modification and improvement;finally,the problems in the implementation process were solved through pre-testing,and the draft of the scheme measurement was formed with 28 entries in 7 dimensions.Part II: A total of 35 study subjects completed 6 months of program application,and the results of the repeated measures ANOVA showed that the differences in quality of life scores(Ftime=2.978,p=0.040),pain scores(Ftime=3.976,p=0.016),and agonistic behavior scores(Ftime=5.941,p=0.003)at different time points were statistically significant(p< 0.05).The results of the Bonferroni pairwise comparison showed that the differences in quality of life and pain scores before the intervention were not statistically significant(p>0.05)and the differences in agonistic behavior scores were statistically significant(p<0.05)when compared with 1 and 3 months of the intervention,respectively;the differences in quality of life,pain,and agonistic behavior scores before the intervention were statistically significant when compared with 6 months of the intervention(p< 0.05).The results of interviews with charge nurses and primary caregivers six months into the scheme application were extracted for each of the five subject terms,and the results of the quantitative and qualitative studies were synthesized to analyze the effectiveness and feasibility of the program application.Conclusion:The Namaste care programme for patients with advanced senile dementia is scientific,practical,and feasible.The long-term application of the scheme can improve patient’s quality of life,reduce pain,and decrease agitated behaviors;the implementation of the scheme can improve nurses’ and primary caregivers’ caregiving skills,improve the relationship between participants,and increase primary caregivers’ satisfaction with nurses without additional costs,and is worth promoting.
