| Purpose: Patients with locally advanced prostate cancer have a worse prognosis than those with localized prostate cancer.For patients with locally advanced prostate cancer,especially those with positive clinical lymph nodes,the treatment is still controversial at present.We will conduct further research on it to guide clinical practice.Methods: Patients diagnosed with clinically positive lymph node adenocarcinoma of the prostate from 2004 to 2015 from the U.S.Surveillance,Epidemiology,and Results Database(SEER database)were divided into surgical treatment group and radiotherapy group according to their treatment methods.Overall survival rates were compared and stratified by age.Results: Age is a significant factor influencing the survival of patients with clinically lymph node positive prostate cancer.Overall,surgery group had better survival than radiotherapy group,despite poorer baseline outcomes for radiotherapy group;But in people over 70,radiation group seems to have similar survival rates as surgery group.Conclusion: For clinically lymph node positive prostate cancer patients,individualized treatment should be selected according to the patient’s age.For patients over 70 years of age,radiation seems to be an alternative to surgery,when consider to the patient’s health and life expectancy.Moreover more prospective trials are needed to confirm this conclusion. |
