Purpose: To understand the true incidence of erectile dysfunction(ED)in patients with benign prostatic hyperplasia(BPH),to understand the risk factors for ED in BPH patients,and to establish a predictive model for high-risk patients..Methods: A retrospective study collected men with BPH diagnosed by physical examination at the Health Management Center of the Second Xiangya Hospital from January 2020 to January 2022.The collected data included anthropometric indicators,blood biochemical indicators,prostate B-ultrasound,erectile function questionnaire and urine output.According to the results of the LUTS questionnaire,the patients were divided into the uncomplicated ED group and the combined ED group according to the presence or absence of erectile dysfunction.Regression analysis was performed on the screened risk factors to establish a predictive model for the risk of ED in BPH patients.BPH patients were divided into mild-moderate LUTS group and severe LUTS group according to the IPSS score,and the comparison between the two groups was carried out to screen the risk factors affecting the severity of LUTS,and to establish a prediction model for the severity of LUTS.Finally,the correlation between LUTS and ED was analyzed and the common pathogenic factors were screened.Results: Age,body mass index(BMI),waist circumference,waist-to-hip ratio(WHR),waist-length ratio(WHt R),systolic blood pressure,TG(triglyceride),HDL-C(high-density lipid)between ED group and non-ED group protein),TG/HDL-c(triglyceride to high density lipoprotein ratio),HDL-c/TC(high density lipoprotein to total cholesterol ratio),LAP(lipid accumulation product),VAI(visceral adiposity index),TYG(Triglyceride glucose index),HSI(hepatic steatosis index),hypertension,fatty liver,DM,CVD,Met S levels were significantly different.The selected indicators are correlated with IIEF-5and IIPS.Using ROC and AUC assessments,TYG had the highest diagnostic value in predicting ED in BPH.TYG remained statistically significant(p < 0.05)after multivariate binary logistic regression analysis after adjusting for confounders.Conclusion: Among middle-aged and elderly patients with benign prostatic hyperplasia,TYG has the ability to predict and diagnose ED,and TYG is an independent predictor of ED. |
