Objective: To study the related risk factors in patients with Crohn’s disease complicated with perianal fistula and the related factors of complex perianal fistula,so as to provide some help and thinking for clinical diagnosis and treatment. Methods: The clinical data of patients with CD diagnosed in the inpatient department of the Second Xiangya Hospital of Central South University from April 2014 to March 2021 were analyzed retrospectively.According to whether perianal fistula occurred, they were divided into perianal fistula group(120 cases)and non-perianal fistula group(78cases).The perianal fistula group was divided into complex perianal fistula(71 cases)and simple perianal fistula(49 cases).Univariate analysis,Spearman correlation analysis, multivariate analysis and ROC curve analysis were used to analyze the general demographic data, diagnostic age, disease location, disease behavior, rectal lesions, extraintestinal manifestations,CRP,ESR and other related data of CD patients. Results: Univariate analysis showed that there were significant differences in gender, perianal abscess, rectal lesions, history of intestinal resection, age of diagnosis, disease location,WBC,MPV,CRP and ESR between the two groups(P < 0.05);multivariate analysis showed that gender, perianal abscess, rectal lesions and location of onset were independent risk factors for perianal fistula. When the endoscopic SES-CD score was greater than 15.5,complex perianal fistula was more likely to occur. Conclusion: Compared with non-perianal fistula patients, most of CD patients with perianal fistula are male, and the lesions are characterized by perianal abscess and rectal lesions, and the lesions are more common in ileocolon and colon segments. For CD patients with perianal fistula, complex perianal fistula is more likely to occur when the SES-CD score is more than 15.5.
