Real-World Effect Of Defocus Incorporated Multiple Segments Spectacle Lens On Slowing Myopia Progression Of Children Based On A Large Retrospective Longitudinal Study

Objective: to evaluate the myopia control efficacy of DIMS lens in a real-clinical environment using data form a large retrospective longitudinal study.Method: A total of 3639 children with DIMS lenses in 9 cities of China(Shenyang,Beijing,Hefei,Huzhou,Chongqing,Changsha,Ningbo,Guangzhou and Shenzhen)and 6838 children with single vision(SV)lenses from Shenyang and Changsha aged 6-16 years with at least one year follow-up interval.Patients were allocated to different follow-up group according to the various follow-up interval.After a 1:1 propensity score matching(PSM),2240 pairs of 1-year follow-up group and 735 pairs of 2-year follow-up group were included to compare the myopia progress between the DIMS and SV groups by t-test.Multiple linear regression equation was used to analyze the factors affecting the therapeutic effect of DIMS lens.Result: After 1:1 PSM,there were no statistically significant differences in age,myopia degree and gender at baseline between the DIMS and SV groups.Assessment of the myopia progression for one year and two years between DIMS and the SV group both yielded significant statistical differences(P <0.001 for both).In the 1-year follow-up group,the mean myopia progression in the DIMS group and the SV group was-0.50±0.43 D and-0.77±0.58 D,respectively,with an average difference of0.27±0.02 D,reducing the myopia progression rate by 35%.In the 2-year follow-up group,the mean myopia progression in the DIMS group and the SV group was-0.88±0.62 D and-1.23±0.76 D,respectively,with an average difference of 0.35±0.06 D,and a 28% reduction in myopia progression.The adjusted difference of myopia progression for one year between DIMS and SV group was 0.32±0.02 D,0.20±0.02 D and 0.07±0.04 D in patients with low,medium and high myopia,respectively.There were both statistical differences between the two groups for patients with low and medium myopia(P <0.001 for both),but no statistical difference for high myopia(P=0.08).The adjusted differences of one-year myopia progression in the younger(6-11 years old)and older(12-16 years old)patients were both statistically significant(younger: 0.28±0.02 D,older: 0.23±0.02D;both P <0.001).Conclusion: Based on large real-world data,wearing DIMS lens over one year did slow myopia progression in majority of Chinese children,but no significant effect in high myopia.
