Objective:To study the role of CT perfusion imaging in the early diagnosis of primary hepatocellular carcinoma(HCC)and the monitoring of tumor recurrence after operation by using 320 row dynamic volume CT for whole liver CT perfusion scanning.Methods:The preoperative and postoperative CT perfusion parameters of 25 patients with primary hepatocellular carcinoma confirmed by pathology after surgery in our hospital were statistically analyzed,and the blood supply characteristics of intrahepatic lesions and tissues were observed.The following perfusion parameters were automatically generated by software for the region of interest(the solid part with the most obvious enhancement of the lesion and the remote normal liver tissue): blood flow(BF),blood volume(BV),hepatic artery perfusion index(HAI),portal vein perfusion volume(PVP),The parameters obtained were divided into 21 cases of typical liver cancer focus group,15 cases of small liver cancer group(diameter ≤2cm),25 cases of distant normal liver tissue group,and 18 cases of early recurrence group.Compare the perfusion parameters between the groups for statistical differences.Results:The perfusion parameters of the solid part of the typical liver cancer group were BF 356.71 ± 104.98 m L/(min · 100 m L),BV 35.74 ± 10.91 m L/100 m L,PVP 14.52 ±3.76 m L/(min · 100 m L)and HPI 0.75 ± 0.13%.The BFs were 178.84 ± 50.69 m L/(min · 100 m L),BV was 25.68 ± 7.98 m L/100 m L,PVP was 21.67 ± 4.55 m L/(min · 100 m L),HPI was 0.53 ± 0.19%.The BF of distant normal liver tissue was 88.34 ± 11.75 m L/(min · 100 m L),BV was 17.67 ± 4.25 m L/100 m L,PVP was 56.63 ± 12.74 m L/(min · 100 m L),and HPI was 0.31 ± 0.12%.In the early recurrence group,BF was 161.22 ± 33.42 m L/(min · 100 m L),BV was 26.13 ± 8.87 m L/100 m L,PVP was 19.31 ± 7.76 m L/(min · 100 m L),HPI was0.55 ± 0.17%.1.The perfusion parameters of BF,BV,HAI increased and PVP decreased in the typical liver cancer group compared with the remote normal liver tissue group(P<0.05).2.Compared with the small hepatocellular carcinoma group,the perfusion parameters of the typical hepatocellular carcinoma group and the small hepatocellular carcinoma group increased in HAI,BF,BV,and decreased in PVP,the difference was statistically significant(P<0.05).3.Compared with the remote normal liver tissue group,the perfusion parameters BF,BV,HAI increased and PVP decreased in the small hepatocellular carcinoma group,the difference was statistically significant(P<0.05).4.The perfusion parameters of BF,BV and HAI in the early recurrence group were higher than those in the normal liver tissue,and the PVP was lower than that in the normal liver tissue.The difference between the two groups was statistically significant(P<0.05);There was no statistical difference between the above parameters and those in the small hepatocellular carcinoma group.Conclusion:320 row dynamic solvent CT perfusion imaging can objectively and quantitatively reflect the angiogenesis and blood flow in hepatocellular carcinoma,and plays an important role in the early diagnosis of primary liver cancer and monitoring the recurrence of tumor after operation.
