Comparative Study On Clinical Efficacy Of Total Hip Arthroplasty Through Anterior And Posterolateral Approaches

Abstract/Summary:
Objective:To compare the clinical application effects of direct anterior approach(DAA)and posterolateral approach(PLA)for total hip arthroplasty from different angles.Methods:According to the method of retrospective cohort study,the clinical data of 30 patients aged 65 to 75 years old who were diagnosed as neck of femur fracture or femoral head necrosis in our orthopaedic surgery department from July 2021 to August 2022 and who had undergone total hip replacement for the first time were collected.The collected clinical cases were divided into DAA treatment group and PLA treatment group according to different surgical methods.Record the patient’s gender,admission age,body mass index(BMI),main diagnosis of admission,surgical method,surgical time,surgical incision length,intraoperative bleeding volume,Harris score at 1,3,and 6 months before and after surgery,VAS score before and after surgery,and postoperative complications,and other clinical data,in order to compare the differences in efficacy and prognosis between the two treatment groups.Results:The DAA treatment group showed significantly lower incision length,intraoperative blood loss,and surgical time compared to the PLA treatment group(P<0.05);There was no significant difference in Harris scores between the DAA treatment group and the PLA treatment group before replacement surgery,but Harris scores were higher than those of the PLA treatment group at 1,3,and 6 months after replacement surgery,with a significant difference(P<0.05);There was no significant difference in VAS scores between the DAA treatment group and the PLA treatment group before replacement surgery,but the VAS scores after replacement surgery were lower than those of the PLA treatment group,with a significant difference(P<0.05);The incidence of postoperative complications in the DAA treatment group was lower than that in the PLA treatment group(6.67% vs 20%),with no statistical significance(P>0.05).Conclusion:Through direct anterior approach for total hip arthroplasty,there is a significant improvement in surgical incision length,surgical duration,intraoperative bleeding,and other aspects,reducing the risk of surgery.At the same time,the incidence of postoperative complications in patients is also significantly reduced,and postoperative recovery is faster,providing a more intuitive guarantee for the quality of life of patients,which is worth promoting.
