Correlation Between Anti-mullerian Hormone And Body Mass Index Levels And Ovulation Induction Efficacy Indicators In Assisted Reproductive Patients

Objective This study aimed to investigate the correlation between Anti-Mullerian Hormone(AMH)and ovulation induction measures In Vitro Fertilization-Embryo Transfer(IVF-ET)in infertile patients with different Body Mass Index(BMI).Methods The clinical data of 473 infertile patients who underwent IVF-ET assisted pregnancy for the first time at the Reproductive Center of Shenyang women’s and children’s Hospital from July 2019 to December 2021 and met the inclusion criteria were collected.All enrolled cases were divided into normal weight group(18.5kg/m2≤BMI<24.0kg/m2)293 cases and overweight and obesity group(BMI≥24.0kg/m2)180 cases,and then each group was divided into two subgroups according to AMH,namely AMH normal group(1.1ng/ml≤AMH≤4ng/ml)and AMH increased group(AMH>4ng/ml).The differences of clinical indicators such as age,years of infertility,basal follicle-stimulating hormone(b FSH),basal luteinizing hormone(b LH),basal antral follicle number(b AFC),total dose of gonadotropin(Gn),number of eggs obtained,number of fertilization,number of MII follicles and other clinical indicators were compared and analyzed,and the correlation between AMH and various indexes in each AMH subgroup of different BMI groups was compared and analyzed.Results In the normal weight group and the overweight and obese group,compared with the general data,except for the b FSH group in the normal AMH group,except for the b FSH group that was higher than the AMH increase group,the number of b LH,b AFC, egg acquisition,MII egg count,fertilization number,2PN number and embryo number in the AMH normal group were lower than those in the AMH increase group.In the normal weight group,except for b FSH,b LH and b AFC(P<0.05),there was no significant difference in other clinical indicators(P>0.05)between the normal AMH group and the AMH increased group.In the overweight and obese group,only b FSH and b AFC differed statistically significantly between the normal AMH group and the AMH increased group(P<0.05),and the differences in other clinical indicators were not statistically significant(P>0.05).In the analysis of ovulation induction effect,the number of eggs gained,MII eggs,fertilization,2PN and embryos in the normal weight group and the AMH normal group in the overweight and obese group were lower than those in the AMH increase group.Among them,in the normal weight group,the number of eggs gained,MII eggs,fertilization,2PN and embryo number were statistically significant between the normal weight group and the AMH increased group(P<0.05).In the overweight and obese groups,there were significant differences in the number of eggs obtained and MII eggs between the AMH normal group and the AMH increased group(P<0.05).There was no significant difference in the remaining indicators(P>0.05).The correlation between the clinical indexes of each group was analyzed,and in the normal weight group,AMH was normal group: AMH was associated with the number of eggs obtained,the number of MII eggs,the number of fertilization,the number of 2PN and the number of embryos,all of which were positively correlated.AMH increase group: AMH was not correlated with the number of eggs obtained,MII eggs,fertilization,2PN and embryos.In the overweight and obese group,AMH was normal: AMH was positively correlated with the number of eggs obtained,MII eggs,fertilization,2PN and embryos.AMH increase group: AMH was not correlated with the number of eggs obtained,MII eggs,fertilization,2PN and embryos.Conclusion Whether AMH level is clinically significant in the process of IVF-ET is mainly related to whether AMH is normal,and there is no obvious correlation with BMI size.When AMH is at a normal level,AMH is correlated with some indicators,and has clear clinical significance for the therapeutic effect of ovulation induction.At high levels of AMH,no significant significance was found.
