Objective: By detecting the expression of Endothelial PAS domain protein1(EPAS1)in Clear Cell Renal Cell Carcinoma,cc RCC)and adjacent tissues and their relationship with clinicopathological features and survival prognosis.Therefore,it provides a new target for the treatment of cc RCC and its feasibility as a prognostic indicator.Methods: The expression of EPAS1 in renal clear cell carcinoma and adjacent tissues was detected by immunohistochemistry(SP).Data were analyzed according to the clinical application data obtained.Chi-square test was used to compare the expression of EPAS1 in cc RCC and adjacent tissues,and the correlation between EPAS1 and clinicopathological features was analyzed.Fifty-eight patients were followed up for 5 years after operation.Cox regression model was used for univariate and multivariate analysis.Kaplan-meier method and log-rank test were used to compare survival rates between groups.The test criterion was(P < 0.05)statistically significant.Results1.EPAS1 was mainly expressed in the nucleus or cytoplasm of renal clear cell carcinoma.The total positive rate of EPAS1 in cc RCC tissues was 74.1%,of which41.4% was medium and high expression,and 32.7% was weak expression.The positive rate of EPAS1 in the adjacent tissues was 16.1%,all of which were weakly expressed,and the difference was statistically significant(P < 0.01).2.The positive expression rate of EPAS1 in PT stage(T1-T2)of cc RCC was68.8%,and the positive expression rate in PT stage(T3-T4)was 100.0%,and the difference was statistically significant(P < 0.05).3.The positive rates of EPAS1 were 75.0% and 72.7% in patients aged > 60 years and ≤ 60 years,and 74.3% and 73.7% in males and females,respectively,with no difference(P > 0.05).4.The positive rates of EPAS1 in the presence or absence of regional lymph node metastasis were 93.3% and 67.5%,and the positive rates in the presence or absence of distant metastasis were 100.0% and 67.4%,respectively,and the differences were statistically significant(P < 0.05).5.The positive rates of EPAS1 were 55.0%,76.4%,87.5% and 100.0% in the TNM clinical staging of cc RCC,respectively.The positive rates of early cc RCC(stage I+II)and late cc RCC(stage III+IV)were 64.9%(24/37)and 90.4%(19/21),respectively,and the difference was statistically significant.6.Survival analysis showed that the survival and prognosis of renal clear cell carcinoma patients with high expression of EPAS1 were poor,and the results were statistically different(P < 0.05).7.The results of univariate analysis showed that the positive expression of EPAS1,lymph node metastasis and distant metastasis were correlated with the prognosis of cc RCC patients(P < 0.05).The risk factors with significant correlation in univariate analysis were included in Cox regression model.The results showed that EPAS1 expression was an independent prognostic factor of cc RCC.Conclusion1.EPAS1 has a higher expression level in cc RCC,which is significantly higher than that in adjacent tissues,thus indirectly proving that EPAS1 plays an indispensable role in promoting the transition process of normal renal tissues to cc RCC.2.EPAS1 expression was associated with PT stage,lymph node metastasis and distant metastasis,but not with age and gender.3.EPAS1 expression level,lymph node metastasis and distant metastasis are negatively correlated with the prognosis of cc RCC,and EPAS1 is also an independent risk factor for the prognosis of cc RCC.4.EPAS1 may be used as a key tumor marker to determine the biological behavior of cc RCC,so as to provide new scientific research ideas for clinical practice and hopefully overcome the heterogeneity of cc RCC to become a safe and effective target.
