Association Between Polymorphism Of Betatrophin Gene And Urinary Albumin Excretion Rate In Type 2 Diabetesmellitus

Abstract/Summary:
Objective:To investigate the correlation between rs2278426 polymorphism of Betatrophin gene and urinary albumin creatinine ratio in patients with Type 2 Diabetes Mellitus(T2DM).Methods:A total of 408 patients were enrolled,including 321 T2DM patients and 183 normal glucose tolerance control group(NC group).According to the Urinary Albumin/creatinine Ratio(UACR),T2DM patients were divided into normal T2DM Albumin/creatinine group(183 cases)and abnormal T2DM Albumin/creatinine group(138 cases).The basic human parameters and biochemical indexes were measured in all subjects.The whole genome DNA was extracted from the subjects,and PCR and RFLP were used to detect rs2278426polymorphism of Betatrophin gene.Partial DNA samples were selected for bidirectional gene sequencing to verify the accuracy of PCR-RFLP genotyping results.Chi square test(χ2 test)was used to compare the frequency differences of genotype and allele and Student-t test was used to compare the differences of biochemical indexes between the two groups.Results:(1)Compared with NC group,NA group was older and had a higher smoking rate.The levels of SBP,DBP,BMI,BUN,LDL-c,TG,FPG,2h PG,HOMA-IR and HbA1c were significantly increased,while HDL-c was significantly decreased.The difference was statistically significant(P<0.05).Compared with NC group,MA group was older and had higher smoking rate,and the levels of BUN,SBP,DBP,BMI,LDL-c,TG,FPG,2h PG,FIns,HbA1c,HOMA-IR and SCr were significantly increased.The levels of HDL-c and e GFR were significantly decreased,with statistical significance(P<0.05).There were no significant differences in gender,drinking history and TC levels among the three groups(P>0.05).(2)Compared with NA group,MA group had higher age,higher levels of SBP,FPG,2h PG,HbA1c,SCr,BUN and HOMA-IR,and lower levels of e GFR.The difference was statistically significant(P<0.05).(3)In NA group,the percentage of CC,CT and TT genotypes were 62.8%,32.8%and4.4%,respectively.In the MA group,the percentage of CC,CT and TT genotypes were49.3%,42.0%and 8.7%,respectively.There was statistical difference in genotype distribution between NA group and MA group,and CT genotype frequency in MA group was significantly increased(P<0.05).Dominant model analysis showed that CT+TT genotype frequency(50.7%)in MA group was significantly increased compared with NA group(37.2%)(P<0.05).The T allele frequency of abnormal albuminuria group was significantly higher than that of normal albuminuria in T2DM group,and the difference was statistically significant(P<0.05).(4)In the total population,SCr and e GFR of CT+TT genotype carriers were significantly increased compared with CC genotype carriers,and the difference was statistically significant(P<0.05).UACR of CT+TT genotype was significantly higher than that of CC genotype,and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis indicated that rs2278426 T allele of Betatrophin gene was an independent risk factor for abnormal albuminuria in T2DM patients(P=0.047,OR=1.757,95%CI=(1.007-3.065).(5)All T2DM patients were stratified by age,course of diabetes,HbA1c,and BMI.Results showed that poor glycemic control(HbA1c>8%)was observed in patients with diabetes course<10 years.Patients with low BMI(<24kg/m~2)who carried CT+TT genotype were more likely to have proteinuria(P<0.05).Conclusion:(1)The rs2278426 genotype distribution of Betatrophin gene was statistical difference between the normal albuminuria group of T2DM and the abnormal albuminuria group of T2DM.The frequency of CT+TT genotype in the albuminuria group of T2DM was significantly increased.(2)The rs2278426 genotype distribution of Betatrophin gene was correlated with T2DM with albuminuria.Compared with the CC genotype,the level of SCr and UACR in CT+TT genotype group was increased,the level of e GFR was decreased.
