BackgroundWith the continuous promotion of China’s "Health China 2030" planning outline,the health care industry has undergone rapid changes,and the evolving nursing planning has put forward higher requirements for nurses’ clinical communication skills.The clinical communication ability of nurses is the ability of nurses to reasonably use various communication skills to communicate with patients in order to promote patient recovery,which is the most important job professional ability.In recent years,with the smooth implementation of quality nursing services,the importance of good communication skills of clinical nurses in improving patient satisfaction and building a healthy and harmonious health care environment has become increasingly prominent.ObjectiveTo explore the current situation of clinical nurses’ communication ability and its influencing factors in tertiary care hospitals,and to analyze the association between the characteristics of different types of nurses and their emotional intelligence and Big Five personality.To provide a reference for the adoption of precise interventions for different types of nursing staff to improve their clinical communication skills,and thus promote the overall clinical communication skills of nurses in China.Method1.4655 front-line clinical nurses nationwide were selected as survey respondents from November 2021 to May 2022 using stratified whole-group sampling.The general information questionnaire,the Clinical Communication Competence Scale(NCCCSN),the Emotional Intelligence Inventory(WLEIS)and the Chinese version of the 10-item Big Five Personality Inventory(TIPI-C)were used to conduct the survey.The clinical communication competence scale was used to analyze the potential profiles of nurses’ clinical communication competence,and the factors influencing nurses’ clinical communication competence were explored by descriptive analysis,one-way ANOVA,correlation analysis and multiple linear regression analysis.2.Based on the data of Study 1,the method of potential category analysis was used to identify nurses’ clinical communication skills into different potential categories and to compare the differences in the distribution of demographic variables among nurses in different potential categories.SPSS 23.0 statistical software and Mplus 8.3 software were used to analyze the data,mainly including potential profile analysis,categorical variables expressed as frequencies(percentages),and chi-square test was used to analyze the differences between groups;Pearson correlation analysis was used to explore the association between nurses’ clinical communication competence category nurses and emotional intelligence and Big Five personality,and P<0.05 was considered statistically significant.Results1.Survey on the current status of nurses’ clinical communication ability and analysis of influencing factors: nurses’ clinical communication ability score was(84.67±14.099),and the six dimensional scores were establishing harmonious relationship(17.83±3.460),listening keenly(16.61±3.080),confirming patients’ problems(16.47±3.080),joint participation(10.06±(3.037)points,delivery of valid information(7.96±1.965)points,and validation of feelings(15.73±3.076)points.The nurse’s emotional intelligence score was(82.80±18.238),and the four dimensions scores were self-emotional assessment(21.62±4.933),emotional assessment of others(20.61±5.101),emotional control(20.57±5.009),and emotional use(20.01±5.025).The five dimensions of the nurses’ Big Five personality scores were extroversion(10.01±2.196),agreeableness(8.87±2.377),dutifulness(9.29±2.188),emotional stability(9.35±2.241),and openness(9.81±2.110).The results of multiple linear regression analysis showed that nurses’ education,years of work experience,emotional intelligence and Big Five personality were the main influencing factors of nurses’ clinical communication competence.2.Potential profile analysis of nurses’ clinical communication ability: The clinical communication ability of nurses obtained from this study was divided into three categories: 7.28% of nurses had relatively low clinical communication ability and belonged to the "low communication ability group";50.63% of nurses had relatively good clinical communication ability and belonged to the "medium Communication ability group";42.09% of nurses with strong clinical communication ability belonged to "high communication ability group".The sum of the two categories of "medium communication ability group" and "high communication ability group" reached more than half,indicating that the clinical communication ability of registered nurses in local tertiary hospitals in China is generally at a high level.The differences in the distribution of age,title,position,years of work,personnel relationship,monthly income,number of night shifts,sleep quality,health status,and participation in training related to nurse-patient communication were statistically significant(P<0.05),while the differences in the distribution of gender,marital status,department,and education were not statistically significant(P>0.05).3.Analysis of the relationship between nurses’ clinical communication ability and their emotional intelligence and Big Five personality: comparing the results of the correlation analysis of nurses in three potential categories showed that the clinical communication ability of nurses in the low communication ability group,medium communication ability group and high communication ability group was positively correlated with nurses’ emotional intelligence and Big Five personality(P<0.05).Conclusion1.4655 nurses surveyed in five tertiary care hospitals had an overall high level of clinical communication skills.Nurses’ clinical communication ability was influenced by factors such as nurses’ working years,emotional intelligence and Big Five personality.2.Nurses’ clinical communication ability can be divided into three potential categories,which are high communication ability type,medium communication ability type,and low communication ability type.Low communication ability nurses are mainly characterized by low emotional intelligence.Nurses in the medium communication competence category are mainly characterized by a high proportion of nurses with high emotional intelligence,Big Five personality of agreeableness,dutifulness,and emotional stability.High communication category nurses were characterized by a higher proportion of nurses with high age,married,high emotional intelligence,Big Five personality of agreeableness,dutifulness,emotional stability,and openness.
