A Study On The Status And Influencing Factors Of Kinesiophobia In Patients With Osteoporotic Vertebral Compression Fractures After Surgery During Hospital--family Transition Period

Objective:This study investigated the current situation of pain,kinesiophobia,dysfunction,general self-efficacy and discharge readiness in patients with osteoporotic vertebral compression fracture(OVCF)during hospital-family transition period,and analyzed the influencing factors of kinesiophobia,so as to provide reference for further targeted measures to reduce the incidence of kinesiophobia.Methods:From March 2022 to December 2022,229 patients after OVCF in a tertiary hospital were selected as the research objects.General information questionnaire,visual analogue scale(VAS),Tampa Scale for Kinesiophobia(TSK),Oswestry disability index(ODI),General Self-efficacy Scale(GSES)and Readiness for Hospital Discharge Scale(RHDS)were used to carry out the questionnaire survey.SPSS26.0 software was used to complete the statistical description,single factor analysis,Pearson correlation analysis and linear regression analysis.P<0.05 indicated that the difference was statistically significant.Results:1.A total of 229 questionnaires were distributed in this study,8 invalid questionnaires were excluded,and 221 valid questionnaires were actually recovered,with an effective recovery rate of 96.51%.Among the 221 patients after OVCF,32.6%were males and 67.40%were females,with an average age of 70.70±7.34 years.2.The score of kinesiophobia on the day of discharge of patients after OVCF was(28.55±10.04),of which 45 patients accounted for 20.36%.The score of kinesiophobia one month after operation was(25.17±8.74).Among the patients with kinesiophobia on the day of discharge,48.89%of the patients did not completely eliminate kinesiophobia one month after operation.3.Age,place of residence,marital status,monthly income,form of medical payment,whether there are chronic diseases,and pain were significantly different in the scores of kinesiophobia on the day of discharge of patients after OVCF(P<0.05);there were significant differences in age,marital status,medical payment form,chronic disease and pain between OVCF patients 1 month after operation(P<0.05).4.The kinesiophobia of patients after OVCF was positively correlated with dysfunction,and negatively correlated with general self-efficacy and discharge readiness.5.Pain,dysfunction,general self-efficacy and readiness for hospital discharge were the main influencing factors of kinesiophobia on discharge day after OVCF,which could explain 75.10%of the total variation(P<0.001).Pain,dysfunction,general self-efficacy and discharge readiness were the main influencing factors of kinesiophobia in OVCF patients 1 month after operation,which could explain 61.60%of the total variation(P<0.001).Conclusion(s):1.The incidence of kinesiophobia on discharge day after OVCF was 20.36%.Among this group of patients,48.89%of patients did not completely eliminate kinesiophobia at 1 month after operation.Therefore,medical staff should not only use a variety of targeted interventions to reduce the incidence of in-hospital kinesiophobia during hospitalization,but also strengthen observation and follow-up,track patients’ long-term kinesiophobia and intervene in time to reduce the incidence of family kinesiophobia.2.The degree of pain and dysfunction is a risk factor for kinesiophobia in the hospital-family transition period after OVCF,and general self-efficacy and discharge readiness are protective factors.Therefore,medical staff can formulate targeted intervention programs according to the influencing factors to reduce the incidence of kinesiophobia,ensure that patients continue to follow medical treatment and effective rehabilitation after discharge,and actively return to the community and family.
