The Influence Factors Of Sleep Disorders In Maintenance Hemodialysis Patients And Red Light Therapy

Objective: Describe the status of sleep disorders in maintenance hemodialysis(MHD)patients,to explore the correlation between sleep status and dialysis treatment,blood biochemical indicators and other factors in MHD patients,and to investigate the effect of red light irradiation therapy on sleep quality in MHD patients.Methods: This study involved 111 MHD patients from a single-center blood purification center.General conditions data and biochemical indicators were collected.Sleep status was assessed by the Pittsburgh Sleep Quality Index(PSQI)scale,pruritus was assessed by the DUO's scale.Analyze the factors that affect sleep quality.Thirty patients with PSQI score >5 were selected and divided into two group,each group has 15 cases.Two-phase crossover trial was used to do red light irradiation therapy and placebo therapy.Analyze the effect of red light irradiation treatment on sleep quality.Results: Among 111 cases of patients with MHD,the mean age was 60.9±15.0 years,male: female=1.27:1,the median dialysis age was 24 months,Poor sleep quality(PSQI>5)was found in 84(75.7%)MHD patients.Univariate analysis suggested that age,albumin,and C-reactive protein were influential factors of sleep quality in MHD patients.Multivariate logistic regression analysis showed that age(OR=0.957,P=0.009)and C-reactive protein(OR=0.043,P=0.043)were influence factors on sleep quality PSQI scores in MHD patients.In this study,there was no adverse event during in vitro red light irradiation.After in vitro red light irradiation,the total PSQI score of the patient decreased,and the pruritus symptom score decreased.The difference in the mean value of the decline was statistically significant(P<0.01).Conclusion: The incidence of sleep disturbance in MHD patients is high,age and hypersensitive c-reactive protein are the main risk factors.In vitro red light treatment is safe and can improve the sleep quality and pruritus of MHD patients.It is beneficial to improve the sleep quality of MHD patients by improving the micro-inflammatory state of MHD patients and using red light therapy.
