| Objective: To observe the clinical effect of percutaneous spinal endoscopic implantation of calcium sulfate loaded streptomycin in lumbar intervertebral space for the treatment of lumbar spinal tuberculosis,and to provide a reference for the surgical treatment of lumbar spinal tuberculosis.Methods: A retrospective analysis was performed from March 2016 to March 2022 in our hospital using percutaneous spinal endoscopic surgery(group A,15 cases)or simple posterior debridement,intervertebral bone grafting,and internal fixation(group B,21 cases)clinical data of 36 patients with lumbar tuberculosis treated.By comparing the operation time,postoperative bed rest time,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),visual analogue scale(VAS),Oswestry disability index score(Oswestry disability index,ODI)and other indicators to evaluate the curative effect.Postoperative follow-up of patients with ASIA classification and imaging data,comprehensive evaluation of postoperative spinal structure and function recovery and bone graft fusion.Results: The follow-up time is 12 to 36 months.The age,gender proportion,course,VAS,ODI,ESR,CRP,COBB corner the two groups of patients have no significant differences(P> 0.05).In group B,the blood volume was 735.71± 144.98 ml.The two groups of patients were significantly improved from VAS,ODI,ESR,CRP,and COBB angle after surgery(P <0.05).Compared with Group B,the operation time of Group A was short,the bed was shorter after surgery,and the VAS score was low in March after the operation(P<0.05).However,VAS,ODI,ESR,CRP,and Cobb angle improvement in the two groups of patients are similar(P> 0.05)during the last follow-up(P>0.05).In addition,the ASIA rating of the two groups of surgical patients during the last follow-up was improved compared with pre-surgery,and intervertebral bone planting has been fused.Conclusion: Percutaneous spinal endoscopic lesion debridement and streptomycin-containing calcium sulfate intervertebral implantation surgery and simple posterior surgery have good therapeutic effects on patients with lumbar tuberculosis.In addition,percutaneous spinal endoscopic surgery has less trauma,no need for indwelling drainage,short postoperative bed rest time,and quick recovery. |
