| Objective: In this study,meridian flow injection acupoint application combined with fructus evodiae hot ironing was used to intervene in elderly stroke patients with Qi deficiency constipation.The effects of this method on constipation symptoms,weekly spontaneous complete defecation frequency,constipation quality of life,TCM syndromes,and the use of constipationsupporting drugs in elderly patients with Qi deficiency constipation were explored,and the effect of time factor on the clinical efficacy of acupoint application combined with fructus evodiae hot ironing was explored.To enrich the clinical application of meridian flow injection theory,and improve the efficacy of acupoint application combined with fructus evodiae hot ironing in elderly patients with Qi deficiency constipation.Method: From December 2021 to December 2022,90 elderly stroke patients with Qi deficiency constipation who were hospitalized in the Department of Rehabilitation Medicine of Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine were chosen as the research objects because they fit the inclusion and exclusion criteria.They were split up at random into group A(constipation routine nursing group),group B(acupoint application combined with fructus evodiae hot ironing group)and group C(meridian flow injection acupoint application combined with fructus evodiae hot ironing group),30 patients in each group.Finally,85 cases were enrolled,after a total of 5 cases dropped out of the three groups,28 instances in group A,29 cases in group B and 28 cases in group C.Group A was given routine nursing of constipation,including diet nursing,life nursing,abdominal massage,pelvic floor muscle exercise and psychological nursing.On the basis of routine nursing of constipation,Group B received acupoint application together with hot ironing of fructus evodiae.The intervention time avoided the midnight-noon ebb-flow at 09:00-11:00,the most vigorous spleen meridian of foot Taiyin.According to the actual situation of the department,it was selected as 14:00-18:00 every day.Evodia hot ironing was 20 min each time,once a day.After the end of Evodia ironing,acupoint application of traditional Chinese medicine was performed.The duration of each application was 4 h,once a day.One week of intervention was a course of treatment,and a total of 2 weeks of continuous intervention.The routine nursing of constipation,fructus evodiae hot ironing and acupoint application drugs,course of treatment,acupoint selection and operation methods in group C were consistent with those in group B.The intervention time was from 09:00 to 11:00 at the time when the spleen meridian of footTaiyin was the most vigorous.Before and after 2 weeks of intervention,constipation symptoms,weekly spontaneous complete defecation frequency(SCBM),quality of life,TCM syndrome scores,and constipation-assisted drugs were evaluated.Results:1.Baseline data comparison : There was no discernible difference in general data,constipation symptom score,SCBM,constipation quality of life and TCM symptom score among the three groups(P>0.05).2.Comparison of constipation symptom scores :(1)Comparison within the group : The three groups’ scores after intervention on each category and the overall score of constipation symptoms were lower than those before intervention,and there was a statistically significant difference(P<0.05).(2)Comparison between groups after intervention : After intervention,the scores of each dimension and overall score of constipation symptoms in group C were lower than those in group B and group A.The three groups were compared by Bonferroni test,and the difference was statistically significant(P<0.05).3.SCBM comparison :(1)Comparison within the group : The three groups’ SCBMs were higher after intervention than they were before to it(P<0.05).(2)Comparison between groups after intervention : After intervention,SCBM in group C was more than that in group B,and that in group B was more than that in group A(P<0.05).4.Comparison of constipation quality of life scores :(1)Comparison within the group : After intervention,there was a statistically significant difference in the three groups’ scores on each dimension and the overall constipation quality of life score(P<0.05).(2)Comparison between groups after intervention : After intervention,the scores of each dimension and total score of constipation quality of life in group C were lower than those in group B and group A.The three groups were compared by Bonferroni test,and the difference was statistically significant(P<0.05).5.Comparison of TCM syndrome scores :(1)Comparison within the group : The scores of TCM syndromes and total scores of the three groups after intervention were lower than those before intervention,and the difference was statistically significant(P<0.05).(2)Comparison between groups after intervention : After intervention,the scores and total scores of TCM syndromes in group C were lower than those in group B and group A.The three groups were compared by Bonferroni test,and the difference was statistically significant(P<0.05).6.Comparison of clinical efficacy : After intervention,group A’s overall effective rate was 42.86%(12/28),group B was 72.41%(21/29),group C was92.86%(26/28),group C performed better than groups B and A,there was a statistically significant difference.(P<0.05).7.Comparison of the use of adjuvant drugs : The incidence of adjuvant drugs in group A was 57.14%,24.14% in group B,and 10.71% in group C.Group C was lower than group B and group A,the difference was statistically significant(P<0.05).Conclusion:1.Meridian flow injection acupoint application combined with fructus evodiae hot ironing can improve the constipation symptoms of elderly stroke patients with Qi deficiency constipation,increase the number of independent complete defecation per week,improve the quality of life,improve TCM syndromes,improve clinical efficacy,and reduce the use of patients ’ assistive drugs.2.For elderly stroke patients with Qi deficiency constipation,the nursing effect of acupoint application combined with fructus evodiae hot ironing in constipation symptoms,weekly independent complete defecation times,quality of life,TCM syndromes,clinical efficacy and the use of cleanup drugs is generally better than that of constipation routine nursing and non-Sishi acupoint application combined with fructus evodiae hot ironing intervention,giving full play to the dual effects of TCM nursing technology and TCM time medicine,so the improvement effect is more significant,indicating that the theory of meridian flow injection has the feasibility and advantages of clinical application,which is worthy of further clinical research and application. |
