Correlation Between ADC Histogram Parameters And Lymph Node Metastasis And Tumor Markers Of Rectal Cancer

Objective:The lymph node metastasis of patients with rectal cancer was analyzed by MRI,and the apparent diffusion coefficient whole volume histogram was used for statistical analysis on the basis of diffusion weighted imaging（DWI）,so as to explore the statistical correlation between the texture analysis parameters of the apparent diffusion coefficient histogram and the CEA index of tumor markers in the pathology of rectal cancer.Methods:Collect the MRI image data of 1 3 0 patients with rectal cancer diagnosed in XXXX People’s Hospital from 2019 to 2021 and without radiotherapy and chemotherapy,as well as the results of relevant pathological diagnosis and tumor marker analysis;The DWI scanning sequence images of patients were processed by omnikinetics software,and the tumor ROI curve was outlined under the guidance of the imaging diagnostic teacher.The ADC histogram texture parameters of the whole tumor volume were calculated by post-processing software to obtain ADC_min,ADC_max,ADC_median,ADC₁₀,ADC₂₅,ADC₇₅,ADC₉₀,akenness,kurtosis and other parameters,The ADC histogram composed of these parameters was compared with the collected tumor markers CEA,CA199,CA72-4 and the increase of immune index Ki-67,tumor size and lymph node metastasis.The ADC value between groups was analyzed by Levene method.The samples with aligned variance were tested by t-test,and the samples with uneven variance were tested by Mann Whitney U.After the analysis of variance of the samples,multiple linear regression analysis was carried out with ADC histogram parameters and CEA,CA19 9 and CA72-4 indicators of rectal cancer tumor markers,so as to draw the correlation conclusion.Results:The results of this study confirmed that ADC_min,ADC_{1 0}and ADC_{2 5}were higher in the group with lymph node metastasis than in the group without lymph node metastasis,and the conclusion was statistically different（P<0.05）;In CA724 high expression group,the values of ADC_mean,ADC₁₀,ADC₂₅and ADC₇₅were lower than those in low expression group,and the kurtosis was higher than that in low expression group;The difference between the two groups was statistically significant（P<0.05）.In the CEA high expression group,the parameters of ADC histogram were lower than those in the low expression group,and the difference was not statistically significant（P>0.05）;In the CA199 high expression group,the values of ADC_median,ADC_mean,ADC_{2 5}and ADC₇₅were lower than those in the CA199 low expression group.The P values were 0.035,0.034,0.001 and 0.040,respectively;In the group with high expression of Ki-67（≥50%）,the values of ADC_medianADC_mean,ADC₂₅and ADC₇₅were lower than those in the group with low expression of Ki-67（<50%）.The difference between the two groups was statistically significant（P<0.05）.After multivariate regression analysis of multiple groups of data,it can be concluded that Ki-6 7 has statistical correlation with ADC_median,ADC_mean,ADC₂₅and ADC_{7 5}in ADC histogram texture parameters（P<0.05）,and among these parameters,ADC_medianhas the greatest influence on Ki-67;CA72-4 is correlated with ADC_max,ADC_mean,ADC_{1 0},ADC₂₅and ADC₇₅in ADC histogram texture parameters（P<0.05）,and ADC_meanhas the greatest influence on CA7 2-4.Conclusion:Among ADC histogram texture analysis parameters,ADC_max,ADC_mean,ADC_median,ADC_{1 0},ADC_{2 5}and ADC_{7 5}have high statistical significance in the clinical diagnosis of rectal cancer;ADC_mean,ADC_{2 5}and ADC_{7 5}have high correlation with rectal cancer tumor markers CA72-4,CA199 and immune marker Ki-67.They can be used as guiding imaging indicators in the analysis of whether rectal cancer has lymph node metastasis and the prognosis of rectal cancer.
