| Background: At present, more and more patients are prone to dental caries due to the irregular arrangement of teeth, mild dental caries will lead to tooth defects, and moderate and severe dental caries may affect the health of dental pulp.Dental caries is the main cause of dental pulp periapical disease, and the most effective treatment of pulp periapical disease is still root canal therapy.Through root canal therapy,we can control root canal infection, repair periapical bone defect and promote lesion healing.Due to the patient’s own uneven teeth or aesthetic considerations, some root canal treatment teeth may also be teeth that need to be moved in orthodontic fixed appliance treatment.Alveolar bone is characterized by pressure absorption.Fixed appliance treatment uses the characteristic of alveolar bone to reconstruct the alveolar bone around the teeth by exerting force on the teeth,so as to achieve the purpose of flattening the teeth, rebuilding the patients’ good occlusal relationship and improving the face.According to existing studies, almost 90% of the teeth of orthodontic patients have different degrees of root resorption. There are many studies on root resorption caused by orthodontic fixed appliance treatment, but the results about the effect of orthodontic fixed appliance treatment on root resorption of root canal therapy are still controversial.Such studies usually take the contralateral living pulp teeth of the same name as a reference to observe the changes of root length of teeth treated with root canal. Therefore, through the retrieval of relevant literature in Chinese and English databases, statistics of the corresponding data, the application of statistical methods for analysis,so as to obtain systematic, objective and comprehensive research results. Objective: To analyze the effect of orthodontic fixed appliance treatment on root resorption of root canal therapy and to provide reference for clinical treatment. Methods: Chinese and English databases such as China National Knowledge Internet,VIP Database for Chinese Technical Periodicals,Wanfang,Pubmed,Google Scholar and Embase were searched by computer. There was no restriction on the publishing language, and the retrieval time is from the establishment of the database to December2022.The main purpose of this paper is to search the literature about the need for orthodontic fixed appliance treatment of teeth after root canal treatment and whether root resorption occurs after orthodontics.Meta analysis was used to study whether orthodontic fixed appliance treatment affected the root resorption of root canal treatment teeth. The main steps include searching the database by two trained researchers according to pre-set criteria, screening the studies that can be included by reading abstracts and the full text, and asking the third researcher for advice if there is any objection. After literature screening and data extraction, the quality of the literature was evaluated, and the data were statistically processed and analyzed by Meta using Revman 5.4 software. The test level was α = 0.05,that is,when P<0.05,the difference between the two groups was statistically significant. Results: A total of 10 articles meeting the criteria were included in this study. The results showed that after orthodontic fixed appliance treatment, compared with the contralateral living pulp teeth of the same name, both teeth had slight root resorption, but there was no significant difference(P>0.05).In the gender subgroup,it was found that the change of root length of male patients after orthodontic fixed appliance treatment was less than that of contralateral living pulp teeth of the same name(P<0.05),while the root length of female orthodontic patients was not different from that of control teeth(P>0.05).Extraction and non-extraction orthodontic fixed appliance treatment had no effect on root resorption of root canal treated teeth and contralateral living pulp teeth(P>0.05).The tooth position did not affect the root resorption length of the orthodontic patients and the control teeth(P>0.05).Conclusion: Orthodontic fixed appliance treatment will not aggravate the root resorption of teeth after root canal treatment, and orthodontic fixed appliance treatment can be performed normally. |
