Background:In recent years,the incidence rate of stroke in China shows a rising trend year by year.The comprehensive application of various treatment technologies and the substantial improvement of medical level have made stroke patients have a longer survival period.However,most post-stroke patients still have mild or severe sequelae,which can be manifested in various aspects such as movement,feeling,cognition,speech,swallowing and psychology.Among them,upper limb and hand dysfunction are the main sequelae of stroke patients.The dysfunction of upper limbs and hands has greatly reduced the ability of daily activities of patients,seriously affected the quality of life and social participation of patients,and caused a great burden on the family and society.It has become a hot and difficult point of rehabilitation treatment in recent years.At present,hand rehabilitation robot and neuromuscular electrical stimulation(NMES)are two treatment technologies widely used in the rehabilitation of upper limb and hand function in clinic.Purpose:In this study,the hand rehabilitation robot and NMES are combined to train the wrist joint,metacarpophalangeal joint and interphalangeal joint of the upper limb of the hemiplegic side of the stroke patients,and explore the rehabilitation effect of the combined application of the two on the overall motor function of the upper limb of the hemiplegic side of the stroke patients,so as to help the stroke patients improve their ability of daily life to the maximum extent and return to society as soon as possible.Method:A total of 68 patients with stroke hemiplegia admitted to the Rehabilitation Department of Northern Jiangsu People’s Hospital in Jiangsu Province from June 2021 to December 2022 were collected,including 40 males and 28 females,aged 41 to 90 years.According to the method of random number grouping,68 patients with stroke were randomly divided into experimental group and control group.There were 34 cases in the experimental group and 34 cases in the control group.Both groups of patients were treated with conventional medicine and rehabilitation training,and the experimental group was treated with hand rehabilitation robot combined with NMES;The control group was treated with NMES on this basis.After collecting the basic data of the patients,the Fugl-Meyer Assessment Upper Limb(FMA-UL),Wolf Motor Function Test(WMFT),and Barthel Index(BI)were used to evaluate the upper limb motor function,functional performance,and activities of daily living of the patients before,4 weeks after and 8 weeks after the intervention.SPSS 26.0 statistical software was used for statistical analysis.Results:A total of 63 patients successfully completed the treatment process and participated in the evaluation.A total of 5 patients in the experimental group and the control group fell off(1 case fell off due to the aggravation of the disease,4 cases fell off due to early discharge).1.There was no significant difference between the two groups in gender,age,course of disease,hemiplegic side of stroke(left/right),and type of stroke(ischemia/hemorrhage)(P>0.05).Before intervention,there was no significant difference in FMA-UL,WMFT and BI scores between the two groups(P>0.05).2.After 4 weeks of intervention,the scores of FMA-UL,WMFT and BI in both groups were significantly improved compared with those before intervention(P<0.05);After 8 weeks of intervention,the scores of FMA-UL,WMFT and BI in both groups were significantly improved compared with those before intervention(P<0.05).After 8weeks of intervention,the scores of FMA-UL,WMFT and BI in the two groups were significantly improved compared with those after 4 weeks of intervention(P<0.05).3.After 4 weeks of intervention,the scores of FMA-UL,WMFT and BI in the experimental group were significantly higher than those in the control group(P<0.05).After 8 weeks of intervention,the scores of FMA-UL,WMFT and BI in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion:On the basis of conventional drug therapy and rehabilitation training,the combination of hand rehabilitation robots and NMES or the application of NMES have varying degrees of improvement effects on the motor function and daily living ability of upper limbs and hands in stroke patients,and the treatment effect of intervention for 8weeks is better than that of intervention for 4 weeks.However,compared to NMES treatment,the combination of hand rehabilitation robots and NMES is more effective in the functional rehabilitation of upper limbs and hands in stroke patients.
