| Objects:Appropriate allocation of health resources in professional public health institutions is the basic condition for ensuring people’s public health.This study analyzes the current situation of health resource allocation of professional public health institutions in the central region,evaluates the equity of its allocation,and provides an evidence-based decision basis for health policy formulation of professional public health institutions in the central region.Methods:Based on the status quo of health resource allocation of professional public health institutions in the central region from 2016 to 2020,this study evaluated the fairness of health resource allocation of professional public health institutions in the three regions of the central region through the methods of Theil index,health resource agglomeration degree,and used GIS spatial analysis to visualize the level of resource agglomeration;Use the GM(1,1)grey prediction model to predict the quantity and fairness of health resource allocation in professional public health institutions in the central region in the next five years.Results:1.The current situation of health resource allocation of professional public health institutions in the central regionThe number of professional public health institutions in the central region in 2020 is 4351 institutions,the number of beds is 99597,the number of licensed(assistant)physicians is 75012,the number of registered nurses is 76,874,and the government health expenditure is 610.937 billion yuan.the number of professional public health institutions is decreasing from 2016 to 2020,and the rest of the resources are increasing.The most professional public health machine health resources in 2020 are all in Henan Province,and the least are all in Jilin Province.Compared to 2016,the fixed base growth rate of the number of professional public health institutions in 2020 is-45.41%,the number of beds is 11.12%,the number of licensed(assistant)physicians is 1.71%,and the number of registered nurses is 24.57%.2.Fairness analysis of health resource allocation of professional public health institutions in central region(1)Fairness Analysis of Thiel IndexIn 2020,the Theil index of various health resources was 0.02,0.02,0.01,and 0.03,respectively.The largest Theil index of health resources was the number of registered nurses,indicating poor fairness;The smallest is the number of practicing(assistant)physicians,which means fairness is better.The trend of changes in the Tael index of various resources from 2016 to 2020 is not significant.Developed regions have the smallest Theil index,followed by underdeveloped regions,while moderately developed regions have the highest Theil index,with the highest Theil index for bed resources.Decomposing the Thiel index,the results show that the contribution rate of inter regional differences in the number of professional public health institutions’ resources is greater than that of intra regional differences,while the proportion of intra regional differences in other resources is greater than that of inter regional differences.(2)Fairness Analysis of Health Resource Agglomeration DegreeBased on the geographical perspective,the clustering of various health resources of professional public health institutions in 2020 are 1.71,1.91,1.69 and 1.76 respectively,and the highest clustering of various health resources in 2020 is in the moderately developed regions,i.e.better equity,and the lowest is in the less developed regions,i.e.worse equity.the clustering of various health resources from 2016 to 2020 is on a decreasing trend;based on the population From the perspective,the clustering degrees of various health resources of professional public health institutions in 2020 are 1.01,1.13,1.00 and 1.04 respectively,with the highest clustering degree of resources for the number of professional public health institutions being the less developed regions,and the highest clustering degrees of resources for the number of beds,resources for the number of licensed(assistant)physicians and resources for the number of registered nurses being the developed regions,and the population-based clustering degrees of the central regions in 2016-2020 The concentration of resources based on population in the central region showed a decreasing trend.(3)Fairness Analysis of GIS VisualizationGIS visualization analysis shows that there are differences in the fairness of resource allocation among different regions in the central region.From a geographical perspective,in 2020,the concentration level of various health resources in professional public health institutions in moderately developed regions was the highest,while in underdeveloped regions it was the lowest;From a population perspective,the resource aggregation level of professional public health institutions in developed regions is the lowest,while the resource aggregation level of professional public health institutions in underdeveloped regions is the highest,while other resources are the opposite.(4)Fairness Analysis of GM(1,1)Gray Prediction ModelThe results of GM(1,1)gray prediction model show that by 2025,the number of medical institutions in the central region is 2315,the number of beds is 109433,the number of practicing(assistant)physicians is 76939,and the number of registered nurses is 102645.The total number of medical institutions 2021-2025 is a decreasing trend,the rest are increasing trend.The concentration of various resources in professional public health institutions in the central region shows a decreasing trend from 2021 to 2025,and the fairness of resource allocation has improved.Conclusion:1.The distribution of health resources in professional public health institutions is uneven due to population size and policy preferences among regions and provinces within the region.Measures such as strengthening government intervention,formulating regional health plans tailored to local conditions,establishing multi-channel public health financing mechanisms,and strengthening government supervision can improve the fairness of health resource allocation in professional public health institutions.2.The fairness of population in the allocation of health resources in professional public health institutions is due to geographical fairness and greater intra regional differences than inter regional differences.We can establish a regional resource sharing mechanism to promote fairness by strengthening regional exchange and learning as well as "Internet plus" informatization construction.3.The fairness of health resource allocation in professional public health institutions has been increasing year by year,but due to policies,population,and other factors,there will be a gap in the number of professional public health institutions and practicing(assistant)physicians in the future.This phenomenon can be changed by deepening the reform of the salary system,emphasizing the development of professional public health institutions,increasing talent cultivation in professional public health institutions,and improving the incentive mechanism for personnel in professional public health institutions. |
