| Objective: To treat sudden deafness with tinnitus,different doses of lidocaine hydrochloride injection were injected subperiosteal in the posterior mastoid of the affected ear,and the comparative analysis of its therapeutic effect was carried out,aiming to provide reference for clinicians to choose a reasonable method of drug administration.Methods: A total of 280 patients with sudden deafness with tinnitus who received inpatient treatment between September 2020 and December 2022 were selected.They were divided into four major groups according to the type of hearing loss: group 1: 70cases(ears)in the low-frequency descending sudden deafness group,group 2: 66 cases(ears)in the high-frequency descending sudden deafness group,group 3: 84 cases(ears)in the flat sudden deafness group,and group 4: 60 cases(ears)in the total deafness sudden deafness group.The groups were further randomly divided into two groups,and the two groups were injected with 5 ml and 10 ml of lidocaine under the ostium of the posterior mastoid in the affected ear.That is,5ml post-auricular injection group: 140cases(ears)and 10 ml post-auricular injection group: 140 cases(ears).Low frequency postauricular injection 5ml group: 35 cases(ear),low frequency postauricular injection10 ml group: 35 cases(ear);high frequency postauricular injection 5ml group: 33 cases(ear),high frequency postauricular injection 10 ml group: 33cases(ear);flat postauricular injection 5ml group: 42 cases(ear),flat postauricular injection 10 ml group: 42 cases(ear);total deafness postauricular injection 5ml group: 30 cases(ear),and the totally deaf postauricular injection 10 ml group: 30 cases(ears).All four groups were given the same treatment: Cipro(flunarizine hydrochloride capsules)5mg orally at bedtime every night,and Mineralon(betahistine mesylate tablets)12mg and vitamin B1(Vitamin B1,VB1)10mg daily after three meals.Systemic administration: 25 ml of Ginkgo biloba extract injection + 250 ml of 0.9% sodium chloride injection.Tinnitus evaluation questionnaire(TEQ)was asked and completed by the same clinician before the initial treatment and after 7 days of treatment,and pure tone audiometry was performed by the same electroaudiometer to calculate the pure tone auditory threshold average(PTA),calculate the postauricular injection side effects The incidence of postauricular injection side effects was calculated and the causes were analyzed,the data were collected and imported into statistical software(spss 25.0)for data processing,and the efficacy of each group was evaluated according to the statistical results.Results: 1.By analyzing the overall data,the general clinical data(age,gender,duration of disease,ear)of each group were compared,and the differences were not statistically significant,proving the existence of clinical comparability(P>0.05);overall there was a statistical difference in the efficacy of hearing and tinnitus treatment between the 5 ml group and the 10 ml group after treatment(P<0.05).2.Low frequency hearing loss group and flat type group: there is a statistical difference in the efficacy of hearing and tinnitus treatment between the 5ml group and the 10 ml group after treatment(P<0.05).3.High frequency hearing loss group and total deafness type group:there was no statistical difference in the efficacy of hearing and tinnitus treatment between the 5ml group and the 10 ml group after treatment(P>0.05).Overall,the relief of tinnitus symptoms and the improvement of hearing were more obvious with 10 ml of injection behind the affected ear compared to 5 ml behind the affected ear.Conclusion: The efficacy of postauricular injection of 2% lidocaine hydrochloride combined with conventional drugs in the treatment of sudden deafness with tinnitus was significant,and the improvement of overall tinnitus symptoms and hearing level was more obvious when 10 ml of lidocaine was injected behind the affected ear compared with 5 ml of lidocaine behind the affected ear,which improved the clinical treatment effect. |
