Clinical Study On Tiaowei-shugan Decoction In Treating Chronic Atrophic Gastritis(Liver-stomach Qi Stagnation And Stasis Syndrome)

Objective: Research on the clinical efficacy and safety of Tiaowei-shugan Decoction in treating patients with liver-stomach qi stagnation combined with blood stasis syndrome of chronic atrophic gastritis,and to study its mechanism of action,so as to broaden the clinical treatment ideas and strategies of such patients.Methods: A total of 70 fit patients were randomly classified into the treatment group(n=35)and the control group(n=35).All patients in the treatment group were treated with Tiaowei-shugan Decoction,which all patients in the control group were administered to Weifunchun.The improvement of clinical symptom scores,the extent of mucosal image alterations during gastroscopy,histopathological changes and safety indexes of patients before and after theary were recorded,and the results were compared and statistically assessed,after 12 weeks treatment.Results:(1)Individual TCM symptom ratings are compared:Before therapy,there was no significant difference in each symptom scores between the two groups.(P>0.05).After treatment,all symptoms in both groups showed improvement(P<0.01).The improvement of epigastric distension pain and tingling pain,hypochondriac distension pain,emotional induced or aggravated symptoms,belching and acid belching in the treatment group was significantly better than that in the control group(P<0.01),while no significant differences were found between the two groups in terms of chest tightness and good breath symptoms improvement(P>0.05).(2)Comparison of total TCM symptom scores: After treatment,the total symptom score of both groups decreased,and the abatement of the treatment group was significantly higher than that of the control group,indicating that the treatment group in improving the clinical symptoms of traditional Chinese medicine better than the control group(P<0.01).(3)TCM syndrome therapeutic effect comparison: The total efficacy rate of the treatment group after treatment was 91.18%,while that of the control group was 85.29%,and the treatment group showed significantly better efficacy than the control group.(P<0.01).(4)Comparison of mucosal efficacy under gastroscopy: After treatment,the treatment group and the control group had efficacies of 88.24% and 73.53%,respectively,with a statistically significant difference(P<0.05),and the treatment group was more effective.(5)Comparison of pathological scores: The pre-treatment pathology scores of the two groups showed no statistically significant difference(P>0.05).After treatment,the scores of pathological tissues in both groups were decreased(P<0.01),indicating that Tiaowei-shugan decoction and Weifuchun could improve the histopathological status of CAG with liver-stomach qi stagnation and blood stasis.The treatment group showed better improvement than the control group in terms of atrophy,chronic inflammation and activity level(P<0.05),but there was no significant difference between the two groups in improving intestinal metaplasia(P>0.05).(6)Comparison of pathological efficacy : After treatment,the pathological efficacy in the treated and control groups was 91.18%and 85.29%,respectively,with statistical significance(P<0.05).(7)Safety index: Both groups of patients had no accessible abnormalities or adverse reactions during treatment.Conclusion: Tiaowei-shugan Decoction in the treatment of chronic atrophic gastritis with liver-stomach qi stagnation and blood stasis can effectively improve clinical symptoms,gastroscopic mucosal images,histopathology.Its mechanism may be related to reducing gastric mucosal inflammation,reducing mucosal damage,regulating immunity,enhancing immune ability,regulating gastrointestinal motility,promoting gastrointestinal emptying,improving gastric wall blood circulation and other functions.
