Objective: To observe the effects of rhubarb peony decoction on inflammatory factors IL-6,IL-10 and IL-6,IL-10,WBC and CRP in the abdominal drainage fluid after acute purulent appendicitis,and to provide clinical data for exploring the mechanism of rhubarb peony decoction in improving the abdominal inflammatory microenvironment after acute purulent appendicitis.Methods: A total of 60 patients diagnosed with acute purulent appendicitis during the operation were collected from the Department of Gastrointestinal and Hernia Surgery(General Surgery)of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from November 2021 to November2022,and the patients were randomly divided into experimental group and control group,with 30 cases in each group.The control group and the experimental group were given conventional treatment such as anti-infection and analgesia,and the observation group was given Chinese medicine nofrying granules after taking drainage fluid and blood 12 hours after surgery.Specific composition: rhubarb 9g,winter melon kernel 30 g,glauconium 3g,peony peel 12 g,peach kernel 9g.The control group and the treatment group were taken from the peritoneal drainage fluid at the 12 th,36th and 60 th hours after surgery,and interleukin-6(IL-6)and interleukin10(IL-10)were measured in the drainage fluid.The control group and the treatment group were taken from fasting blood at the 12 th,36th and 60 th hours after surgery,and IL-6,IL-10,white blood cells(WBC)and creactive protein(CRP)were measured in the blood.Drainage fluid retrieval method: under aseptic operation,5ml empty needle obliquely punctured the external abdominal drainage tube,take the abdominal wall to the head of the drainage bag,and all the drainage fluid in the drainage tube exposed outside the abdominal cavity,about 3-5m L.During treatment,closely monitor the patient’s vital signs and related indications,and intervene in time for adverse reactions.Results: 1.IL-6 and IL-10,IL-6,IL-10,WBC and CRP indexes in the drainage fluid 12 hours after surgery in the two groups were analyzed without significant difference(P>0.05),and there was no significant significance.2.The IL-6 index of drainage fluid in the experimental group was lower than that of the control group 36 hours after surgery and 60 hours after surgery,and there was a significant difference after statistical analysis(P<0.05).3.The blood IL-6,WBC and CRP indexes of patients 36 hours after surgery and 60 hours after surgery in the experimental group were lower than those in the control group,and there were obvious differences after statistical analysis(P<0.05)4.The IL-10 indexes of drainage fluid in the experimental group were higher than those in the control group 36 hours after surgery and 60 hours after surgery,and there were obvious differences in statistical analysis(P<0.05)5.The blood IL-10 indexes of patients in the experimental group were higher than those in the control group 36 hours after surgery and 60 hours after surgery,and there were obvious differences after statistical analysis(P<0.05).Conclusion: The application of rhubarb peony decoction to postoperative patients with acute purulent appendicitis can alleviate the inflammatory response in the abdominal cavity after surgery and improve the postoperative inflammatory microenvironment,and the mechanism may be related to inhibiting the level of pro-inflammatory factors in the blood and drainage fluid of patients and improving the expression of antiinflammatory factors.
