objective: A stable and efficient model of rats with severe acute pancreatitis induced lung injury was established by retrograde pancreatic duct injection of 5% sodium taurocholate,on the basis of this study,the effect of abdominal paracentesis drainage on the lung injury of severe acute pancreatitis in rats and its mechanism were discussed.Methods : 32 specific pathogen free male SD rats were randomly divided into 4 groups,8 rats in each group: sham group(sham group): The rats were anesthetized and open the abdominal cavity,the abdomen was closed after turning the pancreas several times.Severe acute pancreatitis group(SAP group): the SAP rat with acute lung injury models were induced by retrograde injection of 5% sodium taurocholate into the pancreas and bile ducts through a micro infusion pump.Abdominal paracentesis drainage group(APD group): after the establishment of the rat model of severe acute pancreatitis with acute lung injury,indwell a special drainage tube in the abdominal cavity of rats to drainage of ascites produced by the SAP rat.Pancreatitis associated ascitic fluid group(PAAF group): The ascitic fluid of the SAP rats were collected,and injected it into the abdominal cavity of normal rats.Rats were killed 24 hours after the operation,pancreas,lung tissue,blood and ascites samples were collected.observation of pancreas and lung tissue pathological changes by the Light microscopic and their pathology scores,lung wet / dry weight(W/D)were calculation to evaluate the pancreas and lung injury;The expression of MDA and the activity of MPO in lung tissue was detected by spectrophotometer;The contents of amylase,TNF-? and IL-1? in blood and ascites were detected respectively,and the contents of TNF-?,IL-1? and NF-?B in lung tissue were measured;At the same time,the expression of HMGB1 in the pulmonary tissue was detected by Western blotting and immunohistochemistry.Results: The rat in Sham group did not produce ascites,but SAP group has,and a large number of amylase,TNF-? and IL-1? were detected in the ascites.Pathological score of pancreatic tissue in rats with severe acute pancreatitis,and the serum amylase,IL-1? and TNF-? were higher than those in the sham group,the difference was statistically significant(P<0.05).In addition,compared with Sham group,the pathological score,W/D,MPO,MDA,TNF-?,IL-1?,NF-?B content of rat lung tissue in the severe acute pancreatitis group were increased significantly than those in the Sham group,the difference is statistically significant(P < 0.05).Western blotting and immunohistochemical study showed that,in the SAP groups,HMGB1 protein expression in lung tissue was significantly higher than sham group,the difference was statistically significant(P<0.05).Compared with the rats in the severe acute pancreatitis group,the pancreatic pathological score and serum amylase,TNF-? and IL-1? were all decreased in the APD group,and the difference was statistically significant(P <0.05);In addition,pathological score,W / D,MPO,MDA,TNF-?,IL-1?,NF-?B content in the lung tissue of the APD group were also significantly lower than the severe acute pancreatitis group,the difference was statistically significant(P <0.05).Western blotting and immunohistochemical study showed that,The expression of HMGB1 in the lung tissue of APD group was significantly lower than that in the rat with severe acute pancreatitis group,and the difference was statistically significant(P <0.05),But the above indicators are still higher than the sham group,the difference was statistically significant(P <0.05).Compared with the Sham group,the indexes of the pancreatic,lung tissue and blood of the rats in the PAAF group were increased,the difference was statistically significant(P <0.05).conclusion:Abdominal paracentesis drainage can reduce the level of inflammation in the blood of rats with severe acute pancreatitis,and at the same time,relieve the inflammation damage of the lung tissue,which may be related to the drainage of the pancreatitis associated ascitic fluid with large amounts of damage material.
