Objective: To study the characteristics of related indexes of lipid metabolism in pediatric obesity patients with different syndrome types,analyze the correlation and regularity between each syndrome type,and provide scientific basis for clinical diagnosis and intervention of pediatric obesity with different syndrome types.Methods: A total of 122 children with obesity who met the inclusion criteria of traditional Chinese and Western medicine in the pediatric outpatient department or ward of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2020 to December2022 were collected.Clinical data such as gender,age,symptoms and signs of the children were recorded.TCM syndrome classification was conducted based on syndrome scores,and body composition data and related laboratory test indicators were included.The collected medical records were systematically summarized,combined and sorted,and SPSS was used for statistical analysis.The structure of different syndrome types and their correlation with biochemical indexes such as lipid metabolism were statistically analyzed.Results:(1)General statistics: 90 cases were male and 32 cases were female.In terms of age,26 patients were 6 to 9 years old,69 were 9 to 13 years old,and 27 were 13 to 18 years old.The youngest and oldest were 6.1years and 17.1 years,respectively,with an average age of 11.01±2.36 years.(2)Distribution law of TCM syndrome types: from high to low,there were spleen deficiency and dampness-filling syndrome(40.9%),stomach heat-dampness-blocking syndrome(21.3%),spleen and kidney Yang deficiency syndrome(20.4%)and liver qi stagnation syndrome(17.2%).(3)The relationship between TCM syndrome types and general data: in terms of body fat percentage,spleen deficiency and dampness-filling syndrome was higher than liver qi stagnation syndrome and spleen-kidney Yang deficiency syndrome(P<0.05);There were no significant differences in age,sex and BMI distribution(P>0.05).(4)The distribution rule of lipid metabolism level: obesity children with dyslipidemia accounted for 49%;In terms of dyslipidemia,boys were higher than girls(P<0.05),and the abnormal proportion of syndrome type upper spleen-kidney Yang deficiency was the least(P < 0.05).There was no statistical significance in age distribution(P>0.05).(5)Analysis of TCM syndrome types and lipid metabolism indexes: The test values of total cholesterol(TC),apolipoprotein B(Apo B)and low density lipoprotein(LDL)were higher in children with liver stagnation syndrome(P<0.05);The high density lipoprotein(HDL)test value was lower in the children with spleen deficiency and dampness syndrome(P<0.05).The test value of triglyceride(TG)was higher in children with gastric heat-dampness syndrome(P<0.05).There was no significant difference in apolipoprotein A1(Apo A1)(P>0.05).Conclusion: Most of the obese children were boys,aged 9~13 years old.Spleen deficiency and dampness-filling syndrome is the most common,and spleen deficiency and dampness-filling syndrome often has higher body fat percentage.Obese children were mostly accompanied by abnormal lipid metabolism,most of which were boys.In terms of syndromes,spleen deficiency and dampness were the main syndromes,while spleen-kidney Yang deficiency was the least.There is a correlation between TCM syndrome type and a number of lipid metabolism indexes,suggesting that diet,emotion,constitution can affect lipid metabolism level.
