Correlation Between PLR And Premature Coronary Heart Disease In Different Sexes

Purpose:To probe into the dependence between early onset CHD and PLR in men and women,and how to estimate the availability of the severity of coronary vasculature by PLR.Method:A total of 341 male patients(aged<55 years)and female patients(aged<65 years)who were hospitalized in the Affiliated Cardiology Department of Chifeng University from June 2018 to June 2021 and underwent coronary angiography were retrospectively collected.Among them,107 were in the male PCHD group,68 were in the male control group,71 were in the female PCHD group,and 95 were in the female control group.The results of coronary angiography were scored based on the Synthex score,with mild group(score<23 points)and moderate to severe group(score ≥ 23 points),Divide male and female PCHD into mild and moderate to severe groups,including mild group(n=80 cases)and moderate to severe group(n=27 cases)for male PCHD,mild group(n=46 cases)and moderate to severe group(n=25 cases)for female PCHD.Collect baseline data,laboratory examination results,and coronary angiography results from all patients,analyze the influencing factors of early onset coronary heart disease and severe coronary artery disease in male and female,and explore the correlation between PLR and early onset coronary heart disease in male and female,respectively,Evaluate the diagnostic and predictive value of PLR for severe coronary artery disease in men and women with PCHD separately.Result:1.The white blood cell count,neutrophil count,monocyte count,TC,TG,and PLR of the male PCHD group were higher than those of the control group.The proportion of ACEI or ARB drugs taken,lymphocyte count,and total protein were lower than those of the control group.PLR and TC were independent risk factors for male PCHD.The optimal cutoff value for predicting male PCHD by PLR was 131.585,with sensitivity of 56.1%,specificity of 74.2%,and AUC=0.653(95% CI: 0.571-0.734,P<0.05).2.The proportion of white blood cell count,monocyte count,MPV,and hypertension history in the female PCHD group was higher than that in the control group.The history of ACEI or ARB medication use,height,pulse pressure,platelet count,LDL-C,3.and PLR were lower than those in the control group.Hypertension history and average platelet volume were independent risk factors for female PCHD,while PLR,pulse pressure,LDL-C,ACEI or ARB medication use history was a protective factor for female PCHD.4.The proportion of smoking and alcohol consumption,FBS,white blood cell count,PLR,creatinine,proportion of hypertension history,height,weight,hemoglobin,red blood cell count,neutrophil count,and monocyte count in the male PCHD group were higher than those in the female group,while the total protein,HDL-C,MPV,age,and BMI in the female PCHD group were higher than those in the male group.4.Compared with the moderate to severe group,the mild group of male coronary artery disease had higher lymphocyte counts and albumin levels,while the moderate to severe group of FBS and PLR had higher levels.PLR and FBS were independent predictors of severe coronary artery disease in male PCHD.The threshold for PLR to predict severe coronary artery disease in male PCHD was 145.5017,with sensitivity of 66.3%,specificity of 74.1%,and AUC of 0.746(95% CI: 0.636-0.856).5.Compared with the moderate to severe group,the mild group of female PCHD with coronary artery disease had higher lymphocyte count and albumin levels,while the moderate to severe group of FBS and PLR had higher levels.PLR is an independent risk factor for severe coronary artery disease in female PCHD,with a cutoff value of136.6215,sensitivity of 95.7%,specificity of 68%,and AUC of 0.845(95% CI:0.749-0.942).Conclusion:There are gender differences in the correlation between PLR and PCHD.Elevated PLR is an independent risk factor for male PCHD,while elevated PLR is a protective factor for female PCHD.Elevated PLR is an independent risk factor for severe coronary artery disease in male and female PCHD,and the diagnostic cutoff value in male PCHD is higher than that in female PCHD.
