Objective To observe the efficacy of extracorporeal shock wave combined with kines io-cupping in the treatment of shoulder pain patients after stroke under high frequency ultrasound positioning,and to record and compare the characteristics of soft tissue lesi ons around the shoulder of shoulder pain patients before and after treatment with high frequency ultrasound,so as to comprehensively evaluate the effect of extracorporeal sh ock wave combined with kinesio-cupping under high frequency ultrasound positioning on hemiplegic shoulder pain in the patients after stroke.In order to provide new diagn osis and treatment ideas and evaluation methods for stroke patients with shoulder pain in the future.Methods All patients were from the rehabilitation and acupuncture departments of ou r hospital.A total of 54 patients were recruited according to the diagnostic and inclusio n criteria.Because 3 patients dropped off during treatment,51 patients were finally inclu ded and randomly divided into two groups:shock wave group(26 cases)and combined treatment group(25 cases)according to different treatment methods.The treatment of extracorporeal shock wave was performed by physical therapists under high frequency ultrasonic positioning.The duration of extracorporeal shock wave treatment was once a week,for a total of 4 weeks.On the basis of conventional rehabilitation treatment,patien ts in the combined treatment group were treated with extracorporeal shock wave comb ined with kinesio-cupping under high-frequency ultrasonic positioning.The duration of e xtracorporeal shock wave treatment was once a week,for a total of 4 weeks.The treat ment time of exercise cupping was once a day and 5 days a week,for a total of 4 w eeks.Visual analogue scale(VAS),passive range of motion(PROM),upper-limb subscale of Fugl-Meyer assessment scale(FMA-UL),and modified Barthel index(MBI)were e valuated before and 4 weeks after treatment.The scores of VAS,shoulder PROM,FM A-UL and MBI before and 4 weeks after treatment were statistically analyzed and co mpared between the two groups in order to evaluate the clinical effectiveness and ther apeutic advantages of extracorporeal shock wave combined with kinesio-cupping metho d under high-frequency ultrasonic positioning.Results 1.Comparison of general data of the two groups of patients,such as gender,age,course of disease,stroke type,hemiplegic side and duration of pain;VAS,PROM o f shoulder joint,FMA-UL,MBI and ultrasonic image characteristics of soft tissue around shoulder lesions were not statistically significant between the two groups before treat ment,P>0.05,suggesting the two groups of patients were comparable.2.After 4 weeks of treatment,VAS,PROM of shoulder joint,FMA-UL and MBI in 2 groups were significantly improved compared with those before treatment(P<0.05),The VAS,shoulder PROM,FMA-UL and MBI of the combined treatment group were si gnificantly better than those in the shock wave group(P<0.05).3.Comparison of ultrasound image characteristics of the soft tissue lesions surroun ding hemiplegic shoulder between the two groups before and after treatment showed t hat the ultrasound image characteristics of the shock wave group and the combined tr eatment group were significantly improved compared with those before treatment(P<0.05),and after 4 weeks of treatment,the improvement degree of shoulder capsule thick ness in the combined treatment group was better than that in the shock wave group(P<0.05).Conclusion 1.Compared with high-frequency ultrasound-guided extracorporeal shock w ave therapy only,high-frequency ultrasound-guided extracorporeal shock wave therapy c ombined with kinesio-cupping therapy can further alleviate the degree of shoulder pain in patients with post-stroke shoulder pain,improve the passive range of motion of sho ulder joint on the hemiplegic side,and improve the motor function and activity of dail y living in stroke patients with shoulder pain.2.High-frequency ultrasound can effectively record the characteristic changes of ult rasound images before and after treatment of patients with soft tissue lesions around t he shoulder,which can more intuitively evaluate the accuracy and effectiveness of shou lder pain treatment.
