Objective1.To describe the occurrence of symptoms of abnormal urination in patients with post-stroke neurogenic bladder(PSNB)and analyze its influencing factors.2.To explore the effect of traditional Chinese medicine Reambao on clinical efficacy,symptom improvement,bladder function,quality of life and incidence of urinary tract infection in patients with PSNB,in order to provide guidance for the rehabilitation of patients with PSNB.Methods1.Investigative research stage.A total of 240 PSNB patients who were admitted in Hebei Provincial Hospital of Traditional Chinese Medicine from June 2020 to December 2020 were selected.General information questionnaires were used to collect the general data of the patients,and the incidence of symptoms of abnormal urination in the patients was counted.Univariate analysis and logistic multivariate analysis were used to analyze of its influencing factors.2.The implementation stage of the intervention program.A total of 150 PSNB patients who were admitted in Hebei Provincial Hospital of Traditional Chinese Medicine from June 2018 to June 2021 were selected as the research objects,and were designed in accordance with the methods and principles of randomized controlled trials.Strict diagnostic criteria,inclusion criteria,and exclusion criteria were established.A random number table was used.The patients were divided into the intervention group(n=75)and the control group(n=75).The control group was given routine nursing intervention,and the intervention group was given the traditional Chinese medicine Ream Bao intervention on the basis of the control group.50 g each of cloves,cinnamon sticks,Magnolia officinalis,mugwort leaves,etc.and200 g of coarse salt are ground into fine powder and placed in the medicine bag,and placed in the bladder area,lumbosacral region,Qihai,Shenque,Guanyuan and other acupoints 7 d was a course of treatment,which lasted for 2 courses.Statistical software SPSS 25.0 was used for analysis before and after the intervention.Chi-square test was used for enumeration data,and t test was used for measurement data.The clinical efficacy,symptom score changes,bladder function,quality of life and the occurrence of urinary tract infection were compared between the two groups.Result1.Investigative research stage.(1)Among the 240 patients with PSNB,urination was laborious 162 times with a frequency of 67.50%,bladder filling feeling was 95 times with a frequency of 39.58%,and urinary urgency occurred 72 times with a frequency of 30.00%.(2)There was no significant difference in age,education level,marital status,BMI,whether to receive rehabilitation training,and the number of strokes between the two groups(P>0.05).The severity of symptoms in patients with women and lower per capita monthly household income was more obvious.more severe(P<0.05),and the severity of symptom clusters in patients with intermittent catheterization and indwelling catheterization was significantly lighter(P<0.05).(3)The results of Logistic multivariate analysis showed that gender(OR=3.628),per capita monthly household income(OR=2.175),and urination pattern(OR=4.662)were the independent risk factors for the group of abnormal urination symptoms(P<0.05).2.The implementation stage of the intervention program.(1)The total effective rate of symptom scores in the intervention group was 97.30%(72/74)and that in the control group was 82.19%(60/73).The total effective rate of symptom scores in the intervention group was higher than that in the control group,the difference being statistically significant(P<0.05).(2)The symptom scores of the two groups after the intervention were lower than those before the intervention,and the scores of the intervention group were lower than those of the control group,and the difference was statistically significant(P<0.05).(3)The maximum bladder capacity and initial bladder filling volume of the two groups after the intervention were higher than those before the intervention,the maximum bladder systolic blood pressure and the residual urine volume of the bladder were lower than those before the intervention,the maximum bladder capacity and the initial bladder filling volume of the intervention group were higher than those of the control group,and the maximum bladder systolic blood pressure,bladder residual urine volume was lower than the control group,the difference was statistically significant(P<0.05).(4)After the intervention,the Urinary Symptoms Disturbance Score(USDS)score in the two groups was lower than that before the intervention,and the intervention group was lower than the control group,and the difference was statistically significant(P<0.05).(5)The incidence of urinary tract infection in the intervention group was 20.27%(15/74),and the control group was 41.10%(30/73).The incidence of urinary tract infection in the intervention group was lower than that in the control group,and the difference was statistically significant Significance(P<0.05).Conclusion1.The incidence of abnormal urination symptom clusters in PSNB patients is relatively high,and there is a close relationship between gender,per capita monthly income of households,and urination patterns with the occurrence of abnormal urination symptom clusters in PSNB patients,corresponding measures should be taken for targeted intervention.2.The use of traditional Chinese medicine Reambao to intervene in PSNB patients can improve clinical efficacy,relieve abdominal pain,urinary frequency and urgency and other clinical symptoms,improve bladder function and quality of life,and reduce the incidence of urinary tract infections.It is a safe and effective nursing intervention.method is worth promoting.
