| 1.Research objectivesAdolescent idiopathic scoliosis(AIS)is an unexplained,three-dimensional structural deformity of the spine.In addition to causing body surface malformation,AIS can lead to patients with back pain and psychological disorders,decreased activity ability,and even in severe cases,it may compress the heart and lungs,endangering life.AIS,due to its idiopathic,progressive,teratogenic and disabling nature,has become a disease that can seriously affect the physical and mental health of the sick teenagers from all aspects.At present,the incidence of AIS in China has an increasing trend.AIS treatment is mainly surgical treatment and non-surgical treatment.Reasonable non-surgical treatment can improve or even control the progression of scoliosis in patients with mild to moderate AIS.Non-surgical treatments include brace fixation and physiotherapeutic Scoliosis specific exercises(PSSE),also known as body therapy,which is a commonly used conservative treatment.However,the specific content and concept of PSSE are controversial,and the concept and therapeutic effect of PSSE are also controversial in different regions and organizations.An important reason is the lack of high-quality studies demonstrating the effectiveness of PSSE treatment.A commonly used measure of PSSE treatment effectiveness is the change of Cobb Angle based on X-ray.However,X-ray has a large radiation,which has a great impact on the health of the main group of patients with PSSE in their youth and development.Therefore,intensive photographing and timely evaluation of the therapeutic effect of PSSE are not possible.Therefore,this project is using three dimensional morphology analysis technology to PSSE process before and after treatment and follow-up of patients with AIS back three-dimensional appearance shape feature of quantitative analysis,the implementation of high frequency and high quality PSSE treatment evaluation,and then proves the validity of PSSE treatment.The purpose of this study was to evaluate the accuracy and repeatability of the body surface photography system,DIERS Formetric 4D posture assessment instrument,to reduce the number of X-ray images and radiation dose of children,and to provide a scientific basis for clinicians and physiotherapists to evaluate the therapeutic effect of PSSE for AIS in the future,so as to guide the treatment process.2.Research objects and methodsThe subjects were recruited from patients with mild to moderate AIS who underwent PSSE in Shenzhen Adolescent Spine Health Center from January 2021 to October 2021.A total of 142 subjects were included,including 114 females and 28 males,aged between 10-16 years old,with an average age of(13.29±2.57)years old.Have first-class hospital at level 2 or above hospital for 1 times total length of the positive side of the spine X-ray photograph and diagnosed with AIS,lateral bending the Cobb Angle average(18.42 + 17.31)°.After the participants were enrolled,body surface moire images were taken by DIERS Formetric 4D posture assessment instrument to capture body surface parameters and analyze relevant data.3.Research resultsDIERS Formetric 4D posture assessment instrument showed a reliability coefficient(Cronbach’s Alpha)of 0.996 for the measurement of scoliosis using variance analysis of repeated measurements(Empower Stats4.0,Wuhan Yi’er Technology Co.,LTD.).Showed that the DIERS Formetric 4D posture assessment instrument was reliable in calculating the scoliosis Angle of each patient.Pearson test,two-tail test and regression analysis were performed on the four AIS markers,including thoracic Angle,lumbar Angle,thoracic kyphosis Angle and lumbar lordosis Angle,which were measured by DIERS Formetric 4D posture assessment instrument and X-ray film and was statistically significant(P<0.001,significance α=0.001).4.Research conclusions1.DIERS Formetric 4D posture assessment instrument has good accuracy and repeatability;2.The DIERS Formetric 4D posture assessment instrument has some deviation from the full-length anteroposterior and lateral spinal radiographs in evaluating scoliosis,but the numerical trend is strongly correlated,which is objective and feasible to guide the PSSE. |
