Objective:By evaluating the expression of ALT,CD4~+T cells,CD8~+T cells,IL-2,IFN-γand other cytokines under different HBV replication levels in HBV infected persons in Xining area,the correlation between the expression level and viral load was discussed,and the reference was provided for evaluating the prognosis of HBV infected persons.Methods:From June 2019 to June 2021,the inpatients admitted to the Affiliated Hospital of Qinghai University and healthy people without abnormal physical examination in the physical examination Center were retrospectively analyzed.All cases were screened strictly according to the inclusion and exclusion criteria,and according to the Chinese Society of Infectious Diseases of Chinese Medical Association,According to the description of viral load level in The Guidelines for The Prevention and Treatment of Chronic Hepatitis B(2019 edition)jointly formulated by the Hepatology Branch of the Chinese Medical Association,screening cases were divided into quantitative HBV DNA levels:52 patients in the control group(<10~2IU/m L)and 38 patients in the low viral load group(≤10~5 IU/m L).36 patients in the high viral load group(>10~5 IU/m L).General data(age,sex,etc.)of patients were collected,HBV DNA levels of patients in the three groups were detected by real-time fluorescence quantitative PCR,ALT levels of peripheral blood were detected by rate method,T cell subsets and various cytokines were detected by flow cytometry,and statistical analysis was performed on the collected data by SPSS26.0 software.Results:1.There was no significant difference in general information(age and gender)between the low viral load group and the high viral load group and the control group(P>0.05);2.Compared with the control group,ALT,CD8~+,IFN-γ,IL-4 and IL-10 levels were increased in the low viral load group and high viral load group,while CD4~+/CD8~+was decreased,the differences were statistically significant(P<0.05).3.Compared with low viral load group,CD8~+,IFN-γ,IL-4 and IL-10 levels were increased,ALT and CD4~+/CD8~+were decreased,and the differences were statistically significant(P<0.05).4.The expression level of CD4~+in the high viral load group was lower than that in the control group,the difference was statistically significant(P<0.05);5.The Il-2 expression level of the low viral load group was higher than that of the control group and the high viral load group,and the IL-2 expression level of the high viral load group was lower than that of the control group,the difference was statistically significant(P<0.05);6.Quantitative HBV DNA levels were positively correlated with CD8~+T cells,IL-4 and IL-10 levels in control group,low viral load group and high viral load group(r=0.235,P<0.001;r=0.466,P<0.001;r=0.623,P<0.001).Conclusion:(1)The disorder of T cell subsets in HBV infected persons is related to the replication status of HBV DNA.With the increase of HBV DNA level,the disorder of T cell subsets is gradually aggravated.(2)When HBV DNA is in the state of low load in the body,the immune system of the body is promoted and liver cells are seriously damaged.(3)The immune system tends to be inhibited when HBV DNA is in a high load state.(4)HBV DNA level is closely related to the immune state of the body,which can provide a reference for the judgment of the immune state of HBV patients.
