Objective:To investigate the influencing factors of cerebral infarction in patients with atrial fibrillation complicated with diabetes mellitus.Methods:Patients had atrial fibrillation complicated with diabetes mellitus who were hospitalized in the departments of Cardiovascular Medicine,endocrinology and Neurology of Guiyang First People’s Hospital from January 1,2017 to December 31,2021 were collected.Subsequently,patients were divided into cerebral infarction group and non-cerebral infarction group according to the results of cranial magnetic resonance imaging,and the factors influencing the occurrence of cerebral infarction were screened by comparing the characteristics of the two groups.The data of patients was collected including patients’ basic information,medical and medication history,blood biochemical parameters within 24 h after admission,echocardiography,electrocardiogram,cranial magnetic resonance,and the hospital discharge diagnosis.Univariate logistic regression was used to analyze the influencing factors of cerebral infarction in patients with atrial fibrillation and diabetes mellitus.Meanwhile,multivariate logistic regression analysis was used to observe the correlation between the influencing factors and the outcome of cerebral infarction.Results:A total of 159 patients with atrial fibrillation combined with diabetes were included,which contained 36 patients developed cerebral infarction(22.6%)and 123 patients without cerebral infarction(77.4%).The average age of the study population was 76.04 years and was characterised by advanced age,comorbid cardiovascular disease and a general risk of stroke..The results showed significant differences were found that FPG and Hb Alc in cerebral infarction group compared with the non-cerebral infarction group(P < 0.05).After adjusting for confounding factors,including gender,smoking,alcohol,hypertension,coronary heart disease,chronic kidney disease,dyslipidemia,heart failure,BNP,and BMI et al,the multivariate logistic regression analysis revealed that age > 75 years(P=0.006,OR=5.595),fasting plasma glucose > 7.0mmol/ L(P=0.000,OR=8.774),glycosylated hemoglobin >8.0%(P=0.000,OR=7.893)were risk factors for cerebral infarction(P < 0.05,OR> 1).Conclusions:Age,fasting plasma glucose,glycosylated hemoglobin may increase the risk of cerebral infarction in patients with atrial fibrillation combined with diabetes mellitus.Specifically,age > 75 years,fasting plasma glucose > 7.0mmol/L and glycosylated hemoglobin > 8.0% were risk factors for cerebral infarction after adjusting for confounding factors.
