The Expression Intensity Of HLA-B27 Was Correlated With The Clinical Characteristics Of Patients With Spondyloarthritis-single Center Cross-sectional Study

Objective:To evaluate the correlation between the expression intensity of HLA-B27molecular disease-related subtypes and different clinical characteristics of patients with spinal arthritis(Sp A).Methods:A total of 1065 Sp A patients treated in the Affiliated Hospital of Qingdao University from 2011 to 2020 were included as the research object,and their clinical features and CT image classification of sacroiliac joint were analyzed.HLA-B27 detection the expression intensity of HLA-B2702～10,2714 and 2717 subtypes was detected by flow cytometry,which was expressed by fluorescence intensity.U-test,Spearman correlation and logistic regression were used to analyze the correlation between HLA-B27 expression intensity and clinical features,and the correlation between sacroiliac joint CT image score≥2 and clinical characteristics.Results:The intensity of HLA-B27 expression was a protective factor for age at visit(r_s=-0.224,P<0.001),age at onset(r_s=-0.233,P<0.001),BMI(r_s=-0.209,P<0.001),peripheral joint pain(r_s=-0.089,OR=0.956,95%CI=0.927-0.986,P=0.004),sacroiliac joint CT Score(r_s=0.301,P<0.001),family history(r_s=0.156,OR=1.092,95%CI=1.055-1.130,P<0.001),back pain(r_s=0.075,OR=1.088,95%CI=1.016-1.166,P=0.016,P=0.016),smoking history(r_s=0.077,OR=1.041,95%CI=1.009-1.074,P=0.012),drinking history(r_s=0.061,OR=1.032,95%CI=1.000-1.065,P=0.047),NSAID use(r_s=0.141,OR=1.076,95%CI=1.043-1.111,P<0.001),DMAID use(r_s=0.087,OR=1.046,95%CI=1.014-1.079,P=0.005),and the risk factors for hormone use(r_s=0.061,OR=1.033,95%CI=1.001-1.067,P=0.046)and bi DMAID use(r_s=0.067,OR=1.037,95%CI=1.004-1.071,P=028).HLA-B27 intensity(OR=1.222,95%CI=1.146-1.303,P<0.001)was the third risk factor for sacroiliac joint CT image score≥2after men(OR=9.0600,95%CI=5.226-15.709,P<0.001)and inflammatory bowel disease(OR=2.416,95%CI=1.277-4.573,P=0.007).Conclusions:The high expression of HLA-B27 is related to the early age of treatment,low age of onset,high CT Score of sacroiliac joint,family history,low back pain and less peripheral joint pain in this group of spa patients.
