Object: Study the level of IL-23in non-radiographic axialspondyloarthritis(nr-aSpA) patients. Discuss the significance of IL-23level andmagnetic resonance imaging(MRI) changes. To offer more clinical andradiographic basis for pathogenesis, diagnosis and treatment of aSpA.Method:(1) This study included30patients in experimental group with adiagnosis of SIJ-nr-aSpA according to the SpA classification criteria published byAssessment of SpondyloArthritis International Society (ASAS) in2009,30patients in the control group1with a diagnosis of ankylosing spondylitis(AS)according to the Modified New York criteria, and30healthy individuals in thecontrol group2.(2) IL-23amount of each individual was measured by enzymelinked immunosorbent assay (ELISA).(3)Patients in experimental group andcontrol group1were received MRI scan of SIJ, and were scored by a radiologistand a rheumatologist according to Spondyloarthritis Research Consortium ofCanada (SPARCC) scoring system.(4)The data of ESR、CRP、BASDI、BASFI、BASMI、ASDAS、SPARCC scores in experimental groups and control group1were collected, the comparison of IL-23levels and SPARCC scores betweentwo groups were performed, and the correlation analysis of the above data wereperformed.Results:(1) Serum IL-23in SIJ-nr-aSpA was (31.76±6.63) pg/ml;significantly higher than those of AS patients (24.75±6.70)pg/ml andhealthy contro(l22.43±4.34)pg/ml respectively(p<0.001).The serum IL-23in ASwas obviously higher than that of healthy control (p<0.05).(2) No associationbetween individual level of IL-23with CRP,ESR, MRI changes and clinicalscoring (BASFI,BASMI,BASDI and ASDAS) were observed in SIJ-nr-aSpApatients.A positive correlation was observed between individual level of IL-23and clinical scoring BASDI in AS patients.No association between individuallevel of IL-23with CRP,ESR, MRI changes and clinical scoring (BASFI,BASMIand ASDAS) were observed in AS patients.(3)There were MRI changes inSIJ-nr-aSpA patients.Scores according to the SPARCC scoring system in aSpA(18.57±10.55) was a little lower than that of AS patients(19.83±12.59),but therewas no significance(p>0.05).(4). No association between individual Scoreswhich scored by SPARCC scoring system with CRP,ESR and clinical scoring(BASFI, BASMI, BASDI and ASDAS) were observed in both SIJ-nr-aSpA groupand AS roup.Conclusion:(1)IL-23level was increased in serum from aSpA patientscompared with healthy persons. SIJ-nr-aSpA patients was higher compared withAS persons. IL-23played a critical role in the early stage of theaSpA.(2)Sacroiliac joints MRI emerged as a means to diagnosis in thenon-radiographic axial SpA.(3)No association between individual level ofIL-23with MRI changes was observed in aSpA patients.
