Analysis On Current Situation And Influencing Factors Of Patients With Multiple Diseases In A Hospital

Objective: To investigate the present situation of chronic diseases and the coexistence of multiple diseases among inpatients in affiliated Hospital of Youjiang Medical College for Nationalities.Furthermore,the relationship between age,sex,occupation,residence,length of stay,hospitalization cost,BMI,education level,fasting blood glucose,blood lipids and the co-existence of chronic diseases and multiple diseases was further analyzed.So as to guide clinicians to formulate a reasonable treatment plan for patients with chronic and multiple diseases.Methods:By randomly selecting 1855 inpatients from all the patients hospitalized in the Affiliated Hospital of Youjiang Medical College for Nationalities in 2021,their basic information was obtained.The diseases of the patients were classified according to ic D-10,and the data were preliminarly sorted out by Excel software,and the data were statistically analyzed by SPSS26.0.The statistical methods used were Chi-square test,Pearson correlation analysis,Kruskal-Wallis H nonparametric test,Mann-Whitney U test and Logistic regression analysis.Results :(1)among the 1855 people surveyed,690 people(37.20%)were patients with multiple diseases.Among the cases of multiple coexisting diseases,216 were farmers,accounting for 31.30%;Most of the patients were hospitalized for 5-10 days,with 304 patients(44.06%).The proportion of 24 ≤ BMI < 28.0kg/m2 was the highest(310,44.93%).The proportion of patients with junior middle school education was the highest(213,30.87%).499patients(72.32%)with total cholesterol < 5.18mmol/L;There were 538 patients with triglyceride < 1.7mmol/L,accounting for 77.97%;There were 282 patients(40.87%)with high-density lipoprotein 1.04 ~ 1.54mmol/L.The number of people with LOW density lipoprotein < 3.37mmol/L was the largest,492,accounting for 71.30%;Fasting blood glucose< 6.10mmol/L in 422 patients,accounting for 61.16%.(2)There were 433 patients with two chronic diseases,accounting for 62.75% of all patients with multiple diseases.Among all chronic diseases,hypertension,coronary heart disease,diabetes,renal insufficiency/renal dialysis are more common.Among 433 patients with multiple diseases of the two chronic diseases,154 patients with hypertension and coronary heart disease were the majority,accounting for 35.16% of all patients with the two chronic diseases.Among 159 patients with multiple diseases of the three chronic diseases,43 patients with hypertension,coronary heart disease and diabetes were the majority,accounting for 27.04% of the patients with multiple diseases of the three diseases.Among the 98 patients with more than 4 chronic diseases,24 patients with hypertension combined with coronary heart disease combined with diabetes combined with renal insufficiency/renal dialysis were the most common,accounting for24.49% of the patients with more than 4 chronic diseases.(3)According to multivariate Logistic analysis,gender had no effect on the coexistence of multiple diseases,while age,occupation,place of residence,BMI,education level,blood lipid and fasting blood glucose had an effect on the coexistence of multiple diseases.The coexistence of patients with multiple diseases will affect the length of hospitalization of patients;Age was an independent factor influencing the coexistence of multiple diseases.Conclusions :(1)there are many patients with multiple diseases in this hospital,and the diagnosis and treatment mode of multidisciplinary collaboration should be strengthened to deal with many problems caused by patients with multiple diseases.(2)Age,occupation,education level,BMI,blood lipid,fasting blood glucose and the coexistence of multiple diseases.(3)Local clinicians should strengthen management and take more active and effective prevention and control measures to deal with the special population with multiple diseases.
