Objective:This study aims to establish a scientific and practical general clinical practice training program for drug prescribing applicants of clinical nurses in China based on a large number of literature review on clinical practice training of drug prescribing for nurses at home and abroad,combined with the current situation of China’s medical and health environment and the educational background of clinical nurses.The purpose of this study is to provide reference for clinical practice education and training of drug prescribing applicants of nurses in China.Methods:1.Literature research method.By referring to domestic and foreign databases,websites of foreign nursing colleges,websites of foreign nurses’ associations and related books,the development background and practice training status of prescribing rights of international and domestic nurses were sorted out and analyzed.Based on the educational background of clinical nurses in China and the contents and Requirements of Standardized Training for Resident Physicians(Trial)and other documents,a general clinical practice training program for drug prescribing applicants of nurses in China was preliminarily constructed.2.Semi-structured interviewOn the basis of literature review and scientific research group discussion,semi-structured interview content and expert selection criteria were determined.A total of6 experts,including 2 nursing,medical and pharmaceutical experts engaged in clinical and education,teaching and training related work,were included to conduct semi-structured interview.The results of expert interviews were sorted out and analyzed.After group discussion,the structure and items of the questionnaire were revised to determine the first round of formal consultation questionnaire.3.Delphi methodSelected 45 nursing,medical and pharmacy experts from Shanxi,Guangdong,Henan,Nanjing,Chongqing and other places engaged in clinical and educational teaching and training related work,a total of 36 experts participated in two rounds of Delphi consultation.According to the consultation results,the items of the plan were adjusted and revised,and statistical analysis was carried out to establish the final general clinical practice training plan for nurses applying for drug prescription rights in China.Results:1.Analysis of reliability results of expert letter consultationThe average age of the consultation experts was 48.89±7.72 years,and the average age was 24.53 ± 9.86 years.They had rich theoretical knowledge and clinical practice experience,and had good representativeness.The positive coefficients of the two rounds of expert consultation were 80.00% and 100%,the authority coefficients were 0.842 and0.863,and the Kendall coefficients were 0.131 and 0.158(P < 0.001),indicating that the experts were active,their opinions were concentrated,and the consultation results were reliable.2.Construction of general clinical practice training program for nurses applying for drug prescription rights in ChinaAfter two rounds of expert consultation,the general clinical practice training program for applicants for the right to prescribe drugs for nurses has determined 4 first-level items including clinical practice training objectives,training methods,training content and assessment and evaluation,29 second-level items and 84 third-level entries,with a total of117 entries.The specific results are as follows:(1)In terms of training objectives,8 items of 4 competency objectives including nurses’ direct practice ability of drug prescription,nurses’ consultation and guidance ability of drug prescription,nurses’ monitoring and follow-up ability of drug prescription,and nurses’ communication and cooperation ability of drug prescription were specifically covered;(2)In terms of rotating departments and training time in the practical training method,a total training time of not less than 15 months has been determined.At least 1 month),specialist training time is at least 3 months,and the time allocation ratio of outpatient and ward in each department during the training period is 2:1;at the same time,two clinical teaching methods of outpatient teaching and ward teaching have been determined,and from participating in medical investigations quantitative requirements for the clinical workload in the practice training process are made in four aspects: room,in charge of patients,medical record writing and the number of prescriptions issued by specialist practice departments;(3)In terms of practical training content,the practical training content consisting of 1general subject learning content(the content and form of drug prescription for nurses in specific clinical situations)and 15 specialized nurse drug prescription content and prescription forms were determined;(4)In terms of practical training assessment,it covers the assessment and evaluation of the process assessment and the completion assessment.(5)The weight of training program items is set,and the weight of first-level items is practice training content(0.258),practice training method(0.254),practice training objective(0.245),and practice training assessment(0.243).Conclusion:The general clinical practice training program for Chinese nurse applicants for drug prescribing established in this study is scientific,practical,reasonably designed,with high positive coefficient and authority coefficient of experts and reliable consultation results,which can provide a basis for clinical practice education and training of Chinese nurse applicants for drug prescribing.
